The Transitions Team at Holland Bloorview supports the transition of youth with disabilities and their families to the adult healthcare system and a meaningful adult life. Interventions for clients and families are provided in a variety of innovative formats including individual consultations, groups, large events, and partnership programs with community agencies. The Transitions Team is also involved in systems-level changes to bridge pediatric and adult services, and promote joint accountability in meeting client and family needs.
We are looking for a social worker to support “Transition Pop-Ups”. These are innovative group events that bring together clients age 14 years and older, caregivers, and community partner agencies to build capacity in supporting the transition to adulthood gradually over time.
This is a temporary part time (0.6 FTE, or 3 days a week) position expected to be one year in length, contingent upon funding.
**KEY RESPONSIBILITIES**
The Social Worker provides supports related to the transition to adulthood by:
- Conducting brief strengths based assessments of transition readiness and co-creation of goals with clients and families to support the transition-to-adulthood
- Providing goal-oriented group intervention for transition-to-adulthood topics
- Supporting and respecting the client’s/family’s rights to make decisions based on their own values, beliefs and experiences
- Developing and running innovative group intervention and education sessions
- Organizing, coordinating, and executing a large series of transition-to-adulthood events in partnership with adult agency community partners
- Leading partnership and capacity building initiatives with adult health and social care agencies
- Contributing to the academic agenda by supervising multiple student placements per year within a novel Student Led Environment model. This may include supervising interdisciplinary students in partnership with colleagues in occupational therapy, nursing, therapeutic recreation, or other disciplines as applicable.
In addition, social worker will:
- Complete regular client documentation as outlined
- Document accurate workload statistics
- Communicate, collaborate, and meet regularly with the team
- Complete project work to support the development of innovative transition interventions and events
- Act as a representative of Holland Bloorview with adult health and social care agencies in the community by maintaining open, professional communication and a collaborative working relationship
- Remains current with professional literature related to Best Practice Evidence for Transitions to Adulthood
- Participate in quality improvement initiatives, support the maintenance of a safe and healthy work environment and advance a culture of client/patient safety through work and daily practices
**QUALIFICATIONS**
- MSW Degree from an accredited University, Faculty of Social Work
- Current member of the Ontario College of Social Workers and Social Service Workers
- Current member of the Ontario Association of Social Workers preferred
- Previous job related experience working with children and families required
- Previous job related experience working with youth with physical and developmental disabilities within a healthcare context is considered a strong asset
- Knowledge of interventions related to the transition-to-adulthood for clients age 14 years and older, and their families, is an asset
- Must be willing and able to travel to a variety community locations across the GTA
- Must be willing to work hours according to varying requirements of a participant interventions, including evenings and weekends
