**PAY RATE**:$30.39 to $35.97 per hour

**SUMMARY OF THE POSITION**
As the Housing Intake Coordinator (HIC), you will be responsible for the intake, referral processing and the assessment of individuals referred to Thresholds Homes and Supports Inc. (THS). You will work in ways that are consistent with the mission and values of THS. The HIC will respond to inquiry calls, conduct intake assessments, liaise with service providers, obtain collateral information and coordinate service entry in conjunction with the team. You will confirm, through the assessment process, the eligibility of each individual referred and you will be responsible for pertinent administrative activities, including maintenance of the wait list. You will engage in assessment activities with individuals with complex needs, liaise, and collaborate with various departments within Thresholds including Housing Support Services, the Property Department, FACT and ACTT as well as external community partners.
**HOURS OF WORK**
The HIC will work thirty-five (35) hours per week with hours being flexible to the needs of the program. The HIC will participate, on a rotating basis, in the agency’s on-call system.
**ESSENTIAL QUALIFICATIONS**
- Post-secondary degree or diploma in human services/human sciences, or related field.
- At least three (3) years of experience working in mental health, substance use or supportive housing.
- Experience working with individuals who have a mental illness, dual diagnosis, concurrent disorder, and/or acquired brain injury.
- Demonstrated ability of being a highly effective communicator with strong interpersonal and teamwork skills.
- Knowledge of mental health diagnoses, substance abuse, harm reduction, and recovery focused approaches is required.
- Strong assessment skills and demonstrated ability to assess complex situations and make appropriate decisions and/or recommendations independently.
- Sound knowledge of community resources.
- Proficient in the use of Word, Excel, electronic record keeping and data collection systems.
- Certification in First Aid/CPR, Non-Violent Crisis Intervention and suicide risk assessment preferred.
- Must have a valid Ontario G driver’s license, relevant insurance and use of a reliable vehicle.
- Able to successfully complete and provide a current Police Vulnerable Sector Check.
