**The Opportunity**
We are looking for **Connect to Care (C2C) Health Navigator** to join our **C2C/CAMPP Team** at CUPS.
As a **C2C Health Navigator** you will be responsible for delivering trauma-informed case management to participants with complex health and social needs. You will work with participants to develop person-centered care plans that support the unique needs of each individual.
The C2C Health Navigator is a part of an intensive case management team providing care to vulnerable, low-income patients transitioning from acute care to community while advocating for the dignity and voice of patients within the health and social service systems. Under the CUPS Health umbrella, C2C aims to promote equitable access to health and social programs both internally and in the community.
**LOCATION**: Onsite, CUPS - 1001 10 Ave SW, Calgary
**REPORTS TO**: Team Lead, C2C/CAMPP
**CONTRACT TYPE**:Permanent
**SCHEDULE**: Full-time, Mon to Fri, 40 hours per week
**STARTING HOURLY RATE**:$26.88 to $28.21
**Key Responsibilities**
- Provide holistic support to C2C participants including but not exclusive to case conferences in hospital, attending health appointments, participant transport, home visits, creation of care plans, supporting life enrichment and community connection activities
- Advocate for participant needs and guide multi-disciplinary care teams on effective and respectful approaches to working with participants living with the vulnerability of poverty or homelessness
- Perform duties both alone and with colleagues in a variety of settings including in private homes, shelter, on the street, in camps, in office and while transporting participants in personal vehicle
- Provide culturally aware support to CUPS participants, celebrating the diversity of participants and being aware of power dynamics of privilege and oppression that may be present
- Complete health, housing and social needs assessments for referred participants from acute care to formulate individualized care plans that will link participants to needed community resources
- Link unattached participants to an appropriate primary care provider or ensure attached participants have appropriate follow-up with their existing primary care provider
- Ensure all required data is entered accurately into organizational databases, document all appointment notes and referrals provided
- Develop and maintain current Calgary resource and referral knowledge to efficiently navigate and advocate for participant needs
- Coordinate the flow of participant information to internal and external participant care team members for seamless continuation of service in alignment with CUPS Integrated Care service delivery model
**Employment Requirements**:
- 2-3 years’ experience working with people living with the adversity of poverty/homelessness
- 1-2 years experience navigating health care and social service systems
- Basic First Aid and CPR Certification
- Class 5 Driver’s License with drivers abstract and personal vehicle available to use for work purposes & participant transport
- Police/RCMP Background and Vulnerable Sector Check
**What You Bring**:
- Degree or diploma in Social Sciences, Community or Health Sciences preferred
- Knowledge of Calgary homeless sector and Alberta health system
- Demonstrated interdisciplinary team work and patient advocacy skills
- Basic First Aid and CPR Certification
- Brain Story Certification is an asset
- Understanding of trauma-informed care and intergenerational trauma and its effects
**Work Conditions**

- Regular walking, standing, lifting (min 35 lb), and client mobility support
- Use of computers and manual dexterity for clinical procedures
