**The Team**:Toni's House

**The Role**:Case Manager - Part Time (20 hours per week)
- Schedule: Monday to Friday shifts available with flexible hours
- Required to work at least one evening a week for recreation nights and house meetings

**What You'll Bring to The Role**:

- A post-secondary degree or diploma in the social sciences or human services field
- Equivalent combination of education experience and training will be considered
- Minimum 2 years of direct experience working with vulnerable populations
- Knowledge and understanding of the Housing First for Youth & harm reduction models
- Knowledge of the Foyer model's philosophy and operations
- Valid Class 5 Drivers License and Drivers Abstract
- Reliable vehicle as this position requires you to transport youth
- Current First Aid and CPR training
- Excellent verbal and written communication skills
- Able to provide a clear Criminal Record Check with vulnerable sector search and Child Intervention Record Check

**Tasks & Responsibilities**:
Case Management of Youth
- Providing ongoing support in a safe semi-independent home environment which is aligned with program model, guiding philosophies, and best practices
- Intensive Case Management for current youth and alumni within the Toni Kohn House. Case Management responsibilities include, but are not limited to, regular casework meetings, quarterly goal setting, data collection, intakes, discharges, assessment tools, and file management.
- Assist the youth in developing their communication and problem-solving skills
- Support youth in community appointments and meetings, including grocery shopping should they need support
- Support youth at medical appointments or hospital if a mental health or medical crisis occurs
- Facilitate a weekly recreational program and a monthly house meeting.
- Documentation of Reportable Incidents and contacts with the youth within the appropriate timeframe.
- Keep in contact with the youth daily and know of their whereabouts
- Support youth crisis, ensuring safety planning is in place and communicated with the Program Leadership, live-in staff and on-call as required.
- Ensuring chores are completed and completing them if youth are not able or willing, role modeling to youth by ensuring the home is maintained in a clean and orderly state.
- Light to moderate lifting to support youth with their move in's and out's
