The St. James Town Community Corner (The Corner), a multi-service, inclusive hub governed by residents and service providers, is a unique, integrated service model between 20 partner organizations. The Corner is the primary hub for Health Access St. James Town (HASJT) intake, referrals and service coordination. HASJT is a neighborhood based client-focused model of integrated, collaborative care through inter-sectoral service coordination, service enhancement and community development. This initiative is a collaborative between 15 health and social service organizations that form the Health Access Executive Table.

The above is a community centered initiative aimed to enhance interdisciplinary care including primary care, improve access to health, social and community services in St. James Town (SJT), and promotes health and wellbeing in the community. Sherbourne Health and The Neighbourhood Organization co-lead this integrated service model comprising The Corner and HASJT.

**about the role**:
You are a dynamic facilitator who plays a key role in neighbourhood initiatives, community outreach/engagement, peer leadership, and events.

**day to day responsibilities**:
**Peer Engagement, Volunteer & Ambassador Program**:

- Play a lead role in community engagement planning based on resident needs
- Strengthen pathways to ensure that resident interests are integrated into the planning, development and integration of local plans
- Collaborate with community partners to facilitate meetings, working groups, forums and initiatives that further a community needs assessments to support a strategic planning process for resident engagement and service coordination in St. James Town
- Use needs assessments to animate initiatives and program that are responsive to local and emergent needs, and enhance the quality of life of residents.
- Coordinate the consolidation and strengthening of the current community ambassador and volunteer programs in St. James Town
- Enable residents and clients to take facilitator roles in running groups and activities at The Corner
- Create ongoing skill and capacity building opportunities for residents to take on enhanced leadership roles in the community
- Work with community engagement & Outreach staff and with partner organizations

**Outreach & Public Communications**:

- Plan and implement regular community engagement and outreach activities
- Represent The Corner at community events
- Regularly communicate and update multi-modal communication

**Stakeholder Relationship (landlords, businesses, service providers)**:

- Contribute to and participate in various on the ground initiatives, working with community and stakeholders to consolidate and enhance programming
