**Responsibilities**:
**Coordination of Care**:

- Conducting thorough client assessments to develop comprehensive care plans in collaboration with service providers.
- Navigating support for clients and families, ensuring their needs are met on physical, emotional, and cultural levels.
- Advocating for the inclusion of cultural protocols and traditional healing practices in mental health services.
- Ensuring continuity of care and access to necessary services, especially in remote and urban settings.
- Scheduling home visits and client conferences as needed, prioritizing duties to support client care effectively.
- Developing strategies for remote healthcare delivery, including telemedicine, while addressing accessibility challenges.
- Coordinating case conferences with interdisciplinary teams and providing guidance to healthcare providers to ensure quality care.

**Discharge Planning**:

- Facilitating effective communication between mental health staff, healthcare teams, patients, families, and community organizations to ensure clear understanding of discharge plans and available supports.
- Researching and identifying community resources and services to support patients upon their return to the community, such as home care and cultural revitalization initiatives.

**Record Keeping**:

- Creating and maintaining client care plans according to professional standards.
- Recording statistical data and generating reports as necessary.
- Ensuring confidentiality of client information and managing electronic databases effectively.
- Responding to care providers' requests and navigating challenges related to client care.

**Position Requirements**:

- Bachelor’s degree in social work and in good standing with Ontario College of Social Workers and Social Service Workers OCSWSSW OR other allied health professional with a relevant skill set OR Community and Health Services Navigation College Certificate
- 2 years recent related experience in a healthcare, or a combination of related education, training and experience
- Clear Background and Vulnerable Sector Check and proof of immunizations (or valid medical exemption).
- A valid Class G Ontario driver’s license and ability to travel within the region
- Familiarity with mental health services within the region and knowledge of indigenous-specific considerations in care coordination
- A good understanding of the culture and the language is a definite asset
- Familiarity with the independent First Nations Alliance First Nations is also an asset,

**Salary Range**: $75,000 - $86,820

**Schedule**: Regular hours from Monday to Friday with weekends, overtime and on-call availability.

Pay: $75,000.00-$86,820.00 per year
