Summary
The Victoria Youth Clinic Society, lead agency and operator for Foundry Victoria is seeking an Intake Clinician.
In accordance with a client and family centered model of care and, in accordance with the Mission, Vision and Values and strategic directions of the Victoria Youth Clinic Society and Foundry, this position promotes a safe, respectful, and civil working environment for clients, families, visitors and staff. Foundry is removing barriers and increasing access to health and wellness services for young people ages 12 - 24 and their caregivers through integrated mental health care, substance use services, physical and sexual health care, youth and family peer support and social services.
Reporting to the Team Lead and under the direction of the Clinical Coordinator, the Intake Clinician works with youth and families experiencing homelessness, substance use and mental health challenges, and those with difficulties accessing health care services. The clinician works independently, and as part of a robust interdisciplinary team of primary care and allied health care staff to provide advance level clinical interventions. The Intake Clinician coordinates the intake and transition of clients through the continuum of care through assessment, determining referral needs, provides individual support and navigation, education and liaises with community resources. The Clinician helps to formulate treatment plans and provides comprehensive case consultation and follow-up support in collaboration with members of the interdisciplinary team. Participates in local committees, internal and external case conferences and quality improvement activities.
**Qualifications**:
Bachelor’s degree in social work from an accredited University. Eligible for or current registration with affiliated professional body is a requirement
- Minimum two years related experience across the mental health and substance use continuum including experience with interventions and evidence-based treatment modalities;
- Experience working in a fast past interdisciplinary health care team;
- And / or equivalent combination of education, training and experience but priority to those eligible for registration with affiliated professional body.
Skills and Abilities
- Demonstrated ability to provide evidence-informed care to youth and families experiencing substance use and mental health concerns, homelessness and who are adversely affected by social determinants of health.
- Strong understanding of strengths and risks related to issues face by youth and young adults seeking mental health supports, substance use supports and gender affirming care;
- Demonstrated ability to provide care from a trauma-informed, client-centered, harm reduction and Cultural safe practices;
- Experience working with Indigenous, BIPOC, First Nation and LGBTQ2+ communities;
- Demonstrated ability to undertake comprehensive psychosocial assessments that in are formed by theoretical and conceptual frameworks, include mental health and substance use standardized frameworks and experience developing care plans in collaboration with clients and families;
- Ability to make appropriate referrals, reflecting in-depth knowledge of community resources and strong collaborative skills in working with other service providers;
- Ability to work effectively in interdisciplinary team-based settings and independently;
- Effective communication skills, both verbally and in writing;
- Ability to educate and provide consultation to clients, families and other health care professionals;
- Ability to manage complex clinical and psychosocial crisis within identified population;
- Excellent skills in conflict resolution that promotes collaboration and effective problem solving;
- Ability to manage multiple competing demands and organize and prioritize workload;
- Demonstrated knowledge of relevant Acts, including Child and Family Community Services Act,
Mental Health Act and Health Care Consent and Care facilities Act etc.);
- Solid understanding of human services ethics with regard to confidentiality and appropriate boundaries;
- Ability to take initiative and exercise clinical and/or consult with the appropriate teams members to ensure quality of care.
Job Duties
- Conduct comprehensive psychosocial intake assessments using methods such as interviewing client and / or family, gather social data and history regarding client’s family, supports, environment, culture, living situation and financial statues and interpret the data to therapeutically impact the client’s and family’s copying abilities;
- Provides treatment services to youth and families including crisis interventions, short-term counseling support, identifying youth’s goals and expectations, assisting with understanding of treatment recommendations and consulting with colleagues;
