**Job Description**:
In accordance with the Vision, Purpose, and Values, and strategic direction of Island Health (Vancouver Island Health Authority), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Under the supervision of the Program Manager or designate, the Intake Clinician - Foundry provides intake support by receiving referrals from a variety of sources including mental health teams, hospitals, physicians, family members, community agencies and self-referrals; and determining appropriateness of request and client eligibility, to ensure clients meet Foundry program mandates. The Intake Clinician - Foundry provides case management services for clients and their families referred to Foundry programs, and works collaboratively with the multi-disciplinary team to assess care needs and facilitate planning.

**QUALIFICATIONS**:
**Education, Training And Experience**:

- Baccalaureate Degree in a health related discipline from an approved post-secondary institution
- One (1) year recent related experience working with youth and young adults with mental health and/or substance use concerns
- Recognized post basic education related to mental illness and/or substance use
- An understanding of mental health and substance use concerns; assessments, treatment methodologies, intervention strategies, the change process, and community dynamics;
- knowledge of applicable legislation, such as Mental Health and Child Protection legislation;
- knowledge of available programs and services their interrelationships, and their function in delivering services in the community;
or an equivalent combination of education, training and experience.

**Skills And Abilities**:

- Comprehensive knowledge of local and regional resources for youth
- Ability to communicate effectively both verbally and in writing.
- Ability to establish and maintain effective working relationships with clients, families, professional colleagues and other service providers.
- Ability to use professional judgment, empathy, tact, and integrity.
- Ability to make appropriate assessments in situations where the client and family are under stress.
- Ability to take and record social histories and provide short-term counseling
- Ability to counsel in crisis situations.
- Ability to be flexible and adaptable in a fast-paced and changing environment.
- Able to work effectively as an active member of a multidisciplinary team.
