**Duties**:
Under the general direction of the Director and Manager of Patient Care Services, the Mental Health Worker on FACTT (Flexible Assertive Community Treatment Team), a delivery model, that facilitates community living, psychosocial rehabilitation and recovery for people with serious and persistent mental illnesses who have not traditionally benefited from traditional outpatient programs. The Mental Health Worker provides client-centered, recovery-oriented Mental Health Service delivery, and works on FACTT as part of an interdisciplinary team.
As a member of the FACT Team you will:
- Use best-practice and innovative approaches to provide client-centered services including assessment, treatment and psycho-social rehabilitation.
- Provide direct assistance and support to clients regarding substance use, housing, employment, relationships & leisure activities, activities of daily living, and transportation.
- Provide effective intervention, such as motivational interviewing with clients regarding their substance use.
- Establish and maintain effective working relationships with local employers, service providers, stakeholders and families.
- Work collaboratively, cooperatively and effectively within the FACT team and with community partner agencies including hospital inpatient units and emergency.
- Provide support to family and informal caregivers of FACTT clients as needed.
- Provide crisis assessment and intervention to clients on a 24 hour basis (rotating on-call evenings and weekends).
- In collaboration with other FACT Team members and the client, develop individualized client Recovery Plans of Care.
- Assume responsibility for client’s assessments, their treatment, rehabilitation and support of assigned clients within the context of the FACT Team.
- Participate in the design, implementation and evaluation of program services to meet the needs of the program.
- Complete all required non-clinical administrative documentation (i.e.: workload measurement, quality improvement, forms, etc.).
- Participate in daily FACT team meetings to review clients and organize daily assignments.
- Complete and maintain all necessary FACT team documentation within EHR. Documentation maintained in accordance with hospital policies and timelines.
- Advocate with community partners for services and opportunities for FACTT clients.
- Be familiar with the Royal’s strategic plan as well as the goals and objectives of the Community Mental Health Program.
- Keep current with hospital policies and procedures and applicable legislation (Mental Health Act etc.).
- Keep up to date by reviewing current literature, attending workshops/seminars, networking with colleagues and community partners.
- Work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of The Royal.
- Ensure a work environment that is conducive to The Royal’s Anti-Racism, Harassment and Discrimination Free Workplace policy.
**Qualifications**:**
- B.A. in Social or Health Sciences is required.
- Certificate or acceptable combination of education, training and experience in Substance Use, Social Work, Occupational Therapy, Vocational Counselling, Behavioural Therapy or Peer Support is preferred.
- Minimum of 3 years’ experience working with individuals with serious and persistent mental illness
- Minimum of 1 year experience in community mental health is an asset.
- Experience with Psychosocial Rehabilitation principles and practice.
- Knowledge of applicable legislated acts (i.e. MHA, The Health Care Consent Act).
- Excellent communication skills, organizational ability, critical thinking and goal setting skills.
- Excellent observation, assessment and crisis intervention skills.
- Group leadership skills and experience.
- Excellent knowledge of community resources (housing, finance etc.).
- Ability to work effectively as a team member - comfortable with interdependence, consensus decision making and accountability to team needs.
