**WHAT YOU’LL DO**:
Under the direction, supervision and collaboration with the Seniors Care team, Physiotherapist and Occupational Therapist, the rehabilitation assistant for the Assess and Restore+ program will be supporting admitted patients who have been identified in the Emergency Department as requiring a short stay admission who would benefit from goal-focused discharge planning. Patients supported are identified by the OT, PT and GEM Nurse in the Emergency Department and include referred older adults (75+ years of age) who are anticipated to have a short stay admission and require assistance with the navigation of health care services to foster a successful discharge back to the community.
**WHAT YOU’LL BRING**:
- Completion of a recognized Occupational Therapist Assistant and Physiotherapist Assistant Diploma program or equivalent education and demonstrated practice
- Recent relevant experience in the areas of both Physiotherapy and Occupational Therapy
- Recent experience in an acute care setting is considered an asset
- Experience in delivery of both individual and group therapy
**WHO YOU ARE**:
- Knowledge and experience in seniors care.
- Ability to be a strong patient advocate.
- Excellent interpersonal skills and ability to work effectively with staff, physicians and patients/families/caregivers.
- Excellent verbal and written communication skills
- Awareness of availability and access to community resources
- Superior organizational skills to meet workload demands
- Demonstrated ability to be flexible to meet client needs
- Demonstrated ability to work independently or in a group as a team member
- Demonstrated awareness of precautions/contraindications to active treatment
