Coordinator, Urs Respite

The Role:
Reporting the Clinical Supervisor II, the Coordinator, URS Respite works as part of a team supporting children/youth with autism and their families with an identified urgent need to prevent further escalation of risk or harm to a child or youth, other people or property based on the identification of key high-risk factors.
The Coordinator, URS Respite acts as the first point of contact in regard to client respite needs in URS by conducting intake and family support services to clients by conducting needs assessment and developing Individualized Respite Plans to support caregivers in being effective mediators and engage fully in the URS services and supports offered to them.
Core Responsibilities:
- Responsible for conducting comprehensive respite needs assessments with families including the following:
- Provide initial consultation to families and clients to assist them in identifying needs and appropriate respite services/supports
- Support families within the URS to understand and navigate the respite sector including resources and services available
- Assess and determine appropriate services and funding based on eligibility
- Effectively communicate with families and act on behalf of the URS and GCA when providing advice and consultation
- Responsible for developing client-focused, individualized respite service plans for participants based on the needs assessments in order to enable them to access respite programming and services (in home or out of home)
- Accountable for recording and relaying accurate, relevant and timely information to the URS Intake Administrator to ensure seamless intake and booking of clients into GCA URS in centre respite option
- To collaborate with multidisciplinary staff to advocate and support assessment, service options and implementation
- Maintain all records and reports as required by both URS and GCA policies and procedures (and any funder requirements) to include but not limited to case notes and updating information trackers
- Act as a resource contact for external services and clients
- Support families in advocating for appropriate service plans for their children, provides resources and advice.
- Attend community outreach initiatives such as conferences, presentations and fairs
- Develop collaborative relationships and network with related service providers to strengthen external partnerships to help inform families and connect them to programs, services, and resources available in their local communities
- To participate in team and staff meetings regularly and to contribute to program development initiatives within the team and across the GCA
- Other duties as assigned
- University degree in relevant field. An equivalent combination of education and experience will be considered
- At least 3 years’ experience working in a related discipline including providing support to families with ASD
- Previous experience working with families to provide intake and/or service navigation
- Demonstrated knowledge and ability to conduct needs assessments and service plans.
- Previous experience working with children with developmental disabilities and/or Autism
- Excellent verbal and writing communication skills
- Good data collection, filing and report writing skills
- Demonstrated understanding of early childhood education, autism and applied behaviour analysis
- Previous service coordination, planning and advocacy experience
- Demonstrate strength as a team player
- Effective problem solving and judgement skills
- Superior organizational and time management skills
- Ability to work in a flexible work schedule including evenings and weekends when necessary
- Satisfactory current Vulnerable Sector Check and Criminal Records Check
- Proof of full vaccination against COVID-19
Working conditions include:
- Frequent travel in the community to client homes over 66% of the time.
- Frequent phone and computer work
- Outside normal working hours to meet client needs when necessary
