**Position Description**

Reporting to the Director of Dementia Care, you’ll work as part of an interdisciplinary team that includes nursing, music care specialists, and behavioural supports. You’ll support residents in their daily routines, help implement therapeutic music interventions, and use creative strategies to promote autonomy, connection, and high self-esteem, all within a dementia-friendly environment.

**Highlight of Primary Responsibilities**:

- Deliver personal care using a Gentle Persuasive Approach (GPA) and de-escalation techniques.
- Engage residents in meaningful activities, including music-based sessions tailored to their preferences and abilities.
- Collaborate with other staff to implement and evaluate individualized care plans.
- Use DementiAbility resources and techniques to enhance quality of life and promote independence.
- Provide detailed observations to support responsive, coordinated care.
- Participate in Dementia Care team meetings, quality improvement initiatives, and continuous training.

**Qualifications**:

- Must possess certification as a Personal Support Workers through a recognized College or a recognized educational institution. Consideration will be given to those with a valid Health Care Aide Certificate;
- Certification or training in Gentle Persuasive Approaches (GPA), DementiAbility, U-First or PIECES is preferred.
- Demonstrated experience in a long-term care setting is preferable;
- Proficiency in interpersonal communication skills, both written and verbal preferrable;
- Knowledge of dementia types, stages, and symptoms and responsive behaviours an asset.
- Ability to engage residents in music-based activities an asset.
- An understanding of the quality improvement process is an asset
- Experience working with resident living with Dementia and varying mental illnesses is an asset
- Experience completing standardized assessments is an asset
- Employment will be conditional on being fully vaccinated having received both doses of a COVID-19 vaccine and a booster approved by Health Canada.
- Employment will be conditional on providing a current Vulnerable Sector Screening police check, current within 6 months from date of hire.
