Dementia Care Navigator (French Speaking)
- Full Time/ 35 hours per week
The Alzheimer Society of Durham Region (ASDR) is a community support service organization that provides programs and services to people living with dementia and their care partners in Durham Region. We work closely with other service providers to ensure quality care and support for our clients to achieve our mission to improve the quality of life of people living with Alzheimer’s disease or a related dementia and their care partners.
We employ administrative and fund development staff, educators, social workers, social service workers, nurses, occupational therapists, recreation staff, and volunteers who work together as a team to meet the needs of people living with dementia and their care partners.

As a Dementia Care Navigator (French Speaking) (DCN) **, **under the supervision of the Manager, Community Engagement and Training, you will be responsible for supporting persons living with dementia and their care partners within the
French speaking community. The DCN will also be responsible for delivery of education and training to people living with dementia, care partners, healthcare providers and the general public in both official languages. The DCN will be an integral part of a multi-agency approach and community of practice that supports the French speaking community. They will maintain working relationships with clients, professional colleagues, community agencies, and other organizations and groups working with people living with dementia in Durham Region.
What you will be doing
Support (50%)
- Conduct initial intake screening (in French) to identify current and future needs, goals and level of risk of people with dementia and their care partners
- Provide timely and appropriate dementia specific supportive consultation (resource sharing, navigation of healthcare system, linking to programs/services, education, in the form of phone, virtual, home, community, office consultation)
- Provide dementia-related consultation and support with collaborative partner Les Centres d'Accueil Heritage
Adult Day Program (ADP) settings or in home addressing areas of concerns such as responsive behaviours, caregiver stress, and difficulties with activities of daily living
- Coordinates peer to peer and group support sessions for members of the French Language community
Education, Training and Outreach (50%)
- Collaborate and promote connections within a community of practice and continue to strengthen connections within the French Language service delivery agencies and the Durham Region French Language population
- Promotes ASDR programs and services within the French Language community and to community partners who provide service within the French Language community
- Assist in the coordination, design, delivery and evaluation of public education and awareness events, primarily in
French, to a broad variety of target groups, including persons with dementia, family and professional caregivers,
general public, students, volunteers, and community partners using a wide array of delivery methods including, but not limited to, presentations, forums, consultations, dementia simulations, and web-based e-learning.
- Act as resource for inquiries from the general public and for the Society regarding educational programs and services offered in French and English
Other Duties
- Provides French language translation support within all service areas of ASDR, including translating and reviewing resources, documents and other materials as required.
- Participates in and delivers French content for agency events as required
- Maintains a current knowledge of Alzheimer’s disease, related dementias and current resources
- Actively participates in community agency meetings, including representing ASDR on French Language Health
Planning tables
- Completes administrative and data entry for statistical reporting purposes and appropriate documentation of client interactions
- Perform other duties consistent with the job classification, as required.

- Post-secondary degree/diploma/certificate in health, social work, social services, or recreation
