**Overview**
We are seeking a compassionate and dedicated Support Worker to join our team. In this role, you will provide essential care and support to individuals in various settings, including group homes, assisted living facilities, and private residences. Your primary focus will be on enhancing the quality of life for those you assist by promoting independence and dignity while ensuring their safety and well-being.

**Responsibilities**
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation.
- Provide patient care and support in accordance with individualized care plans.
- Monitor vital signs and report any changes in clients' health status to appropriate personnel.
- Ensure a clean and safe living environment by performing light housekeeping tasks.
- Support clients with mobility needs, including the use of Hoyer lifts when necessary.
- Engage clients in meaningful activities that promote social interaction and cognitive engagement.
- Administer medication as prescribed and ensure compliance with HIPAA regulations.
- Document patient observations and care provided accurately in client records.
- Collaborate with healthcare professionals, family members, and other caregivers to deliver comprehensive care.

**Skills**
- First Aid and CPR certification.
- Experience working in group homes or assisted living environments.
- Strong patient observation skills to identify changes in behavior or health status.
- Familiarity with memory care, Alzheimer’s care, dementia care, and hospice care practices.
- Ability to perform heavy lifting as needed for client mobility support.
- Experience working with individuals with developmental disabilities or autism.
- Knowledge of home health care practices and caregiving techniques.
- Proficiency in cooking and meal preparation tailored to dietary needs.

Pay: $18.00-$22.00 per hour
