Case Manager (Alluriarniq)
Full Time, Term end date March 30, 2025 with possibility of extension.
It is the mission of Tungasuvvingat Inuit (TI) to provide Inuit-specific programs that contribute to the health and wellbeing of Inuit, and to encourage and support similar programs for Inuit across the country. With over 30 years of highly successful experience in crafting the design, development, and delivery of a wide range of effective, client-centered services, TI continues to deliver quality programs and services that directly benefit our growing urban Inuit population.
For the Alluriarniq Case Manager, accountability involves providing consistent and client-centered support to Inuit individuals involved in the program. The role requires adherence to Tungasuvvingat Inuit's principles, policies, and procedures while delivering culturally appropriate services. The case manager must be accountable for offering compassionate and professional guidance, ensuring clients have access to the resources they need, such as social services, health care, housing, and legal support.
Maintaining accurate and up-to-date records of client interactions, progress, and service plans is a key accountability measure. The case manager is also responsible for developing individualized plans that address the specific needs and goals of each client, regularly reviewing and adjusting these plans in consultation with the client to ensure ongoing relevance and effectiveness.
Accountability extends to maintaining confidentiality and respecting the cultural values and practices of Inuit clients. The case manager must also engage in continuous professional development to improve their skills and remain informed about best practices in case management, especially concerning Inuit-specific services and trauma-informed approaches.
The case manager should actively collaborate with other service providers and advocate on behalf of clients to facilitate their access to services and resources. Regularly reporting outcomes and challenges to the program coordinator or supervisor ensures transparency and alignment with organizational objectives. Ultimately, the case manager's accountability is reflected in the quality of support provided, the trust built with clients, and the outcomes achieved in promoting the well-being of those served.
**Responsibilities**:
- Provide individualized, culturally appropriate support to clients, developing and maintaining detailed service plans based on their needs, goals, and progress. Regularly review and adjust these plans in consultation with clients to ensure effectiveness and relevance.
- Maintain accurate, up-to-date records of client interactions, service plans, and progress while ensuring strict confidentiality and adherence to Tungasuvvingat Inuit's privacy policies.
- Act as an advocate for clients, helping them navigate social services, legal systems, healthcare, housing, and other resources. Facilitate access to these services by building relationships with external service providers.
- Work closely with other Tungasuvvingat Inuit programs, community partners, and external agencies to coordinate comprehensive support for clients. Participate in case conferences and team meetings to share relevant information and strategies.
- Engage in ongoing training to stay informed about best practices in case management and Inuit-specific services. Regularly report on client outcomes, challenges, and progress to supervisors to ensure alignment with program objectives and continuous improvement.
**Qualifications**:
Completion of Secondary School or Community College, in SSW, Community justice Services, Addictions and Mental Health, Indigenous Community and Social Development is required. Undergraduate Degree in BSW, Psychology, Indigenous Studies, Sociology, Criminal Justice or Criminology and Health Services is preferred. First Aid, CPR, Mental Health First Aid, ASIST, Crisis Intervention and Prevention is required.
One plus years’ experience is required in working with Indigenous Communities - specifically Inuit Communities, Mental and Health Addiction Support, Advocacy and Resource Navigation. Two plus years’ experience is required in Case Management or Social Service Experience, Crisis Intervention and Conflict Resolution.
Salary and Benefits
- Salary is $63,000 annually
