We are currently seeking a passionate and _experienced leader _to join our team as Moments Care Coordinator for our Forest Valley Terrace residence. This community serves seniors in _full assisted living and memory care support through to end of life_.

The Moments Coordinator manages the overall operations of the Moments Neighbourhood and has primary responsibility for meeting the direct care and life enrichment needs of the Moments Neighbourhood residents. The Coordinator has direct supervisory responsibility for the Moments Program Care Managers and is a champion and resource in the Community for all elements and components of the Moments Program.
Job Requirements:
- Proactively schedules and maintains staffing of the Moments Neighbourhood based upon the occupancy and specific care needs of the current residents so as to ensure appropriate levels of staffing, while managing within the budgeted guidelines
- Reviews, modifies and approves direct care labour hours using ADP computer time and attendance software system
- Develops and maintains the Designated Care Manager (DCM) assignments, balancing and reassigning staff as resident census and care needs change
- Trains and supervises care staff assigned to the Moments Neighbourhood in providing day to day resident care in compliance with approved SSL programs, policies and procedures
- Delivers Moments training and orientation programs to new Community employees and ensures all Moments Neighbourhood staff are trained and provide care and life enrichment according to the prescribed Program elements and signatures.
- Actively participates in the resident assessment and care planning process, working closely with the Wellness Coordinator and Executive director, based on the family interviews and ongoing evaluation of resident needs.
- Successful attainment of Health Care Aide (HCA) Certificate or equivalent
- 2-3 years experience providing direct care, preferably to seniors
- Prior supervisory experience desired
- Knowledge and understanding of Alzheimer’s Disease and related dementias and a desire to serve residents and families of the Moments Neighbourhood
- Leadership skills and ability to coordinate care across disciplines (including Wellness, Activities, Dining, etc.)

- You must be able to work with Dogs and Cats and other animals, allergies or other needs cannot be accommodated.
- Due to our high level of residents with dementia; your tattoos and piercings must either remained covered by clothing or are discrete. Any other external body decoration, including hair, must not be frightening to a frail senior with dementia.
- You must be legally allowed to work in Canada
- You must be in good physical health as this job requires lifting, sitting, standing and walking.
- Must be able to read and write in English and basic ability to converse in French
- You must own your own car or have regular access to a vehicle.
- Available on call, weekend, evenings, nights and in emergency situations as required.
**Salary**: From $45,000.00 per year

**Experience**:
- supervisory/management: 2 years (preferred)
