**JOB SUMMARY**:

Under the direction of the Director of Care, the Care Coordinator works collaboratively with the multi-disciplinary care team to establish and maintain a pleasant, safe and homelike atmosphere for residents by effectively coordinating day-to-day operations and facilitating daily clinical activities. The Care Coordinator supervises assigned staff, serving as a role model and coach to assist with professional development and the achievement of service goals, and acts as a clinical resource to nurses and other members of the care team. Ensures effective use of resources by contributing to the development and monitoring of the operating plan. Participates in, and provides leadership and support for, quality improvement and risk management activities.

All staff have a part to play in preventing adverse events and in promoting safety for residents and staff by being aware of and adhering to all policies and procedures including promptly reporting any risks or safety concerns relating to resident or staff safety.

**QUALIFICATIONS**
**Education, Training, and Experience**
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Three (3) to five (5) years recent, related experience in complex care services comparable to those provided by Providence Living Society including up to one (1) year of supervisory experience or an equivalent combination of education, training and experience.

**Skills and Abilities**
- Comprehensive knowledge of the theories/concepts/principles relevant to complex care.
- Demonstrated leadership skills with the ability to effectively supervise staff and guide clinical tasks.
- Demonstrated ability to mentor and coach others including employing conflict resolution skills when needed.
- Demonstrated ability to successfully manage multiple priorities and to coordinate and prioritize the work of others.
- Demonstrated critical thinking and problem-solving skills supported by a systems approach.
- Excellent written and oral communication skills.
- Commitment to Providence Living’s professional and organizational vision for Home for Us as it pertains to clinical practice and resident care.
- Ability to operate related equipment.
- Physical ability to carry out the duties of the position.

**DUTIES**
- Oversees the day-to-day delivery of resident care, aligning the coordination of care activities with Providence Living’s Home for Us philosophy. Monitors resident acuity; consults and reviews resident documentation with interdisciplinary team members; and connects with family members and residents as the initial contact in the delivery of care. Collaborates in the planning and prioritizing of nursing inventions including implementation and evaluation of these measures. Assists in determining the requirement for consultations with Physicians and with other disciplines or departments to assess and/or address resident/family concerns and needs.
- Assesses and adjusts staffing needs to meet planned resident activities and respond to changes in acuity, ensuring appropriate resources are available to meet care requirements. Collaborates with an interdisciplinary team to ensure timely admission and discharge of residents and appropriate optimization of services with the Providence Living Home for Us approach to care.
- Provides leadership and supervision to assigned staff by assisting in the recruitment, orientation, development, and performance evaluation of staff including participating on selection panels; providing input into hiring decisions; determining orientation and training requirements; and giving feedback to the Director regarding performance during the probationary period.
