At Sinai Health, we care, create possibilities and offer hope. We are looking for an experienced safety and risk professional, with the skills and abilities to assume the exciting, System-wide role of **Risk and Safety Specialist.**
Reporting to the Senior Manager, Risk Management, the Risk & Safety Specialist will assist in managing a comprehensive enterprise risk management system through promoting a culture of safety throughout the organization (clinical and non-clinical). This will involve gathering and analyzing patient safety and risk data for patterns and trends in delivery of care and services, organizational day to day operations to identify gaps, risks and areas for improvement. As a key interprofessional partner, the Risk and Safety Specialist works in collaboration with managers, teams and additional stakeholders to support the implementation of quality and risk mitigation solutions.
**Responsibilities**
**Safety Event Reporting System (SAFER)**
- Assists in maintaining, and monitoring the safety event reporting system on a daily basis.
- Reviews incoming safety reports for completion and accuracy and flags issues for follow up with the most responsible person.
- Reviews patient safety data to identify trends and actionable items to advance a just culture.
- Assists in the preparation of reports in the form of tracking, trending, graphs or analyses regarding patient safety events.
- Manages policies and procedures for safety reporting (noncritical and critical) including purpose, definition reportable incidents, documentation, archiving, analysis and follow-up of safety reports.
- Reviews all incidents and complaints representing risk exposure to the Sinai Health.
- Ensures timely reporting of required events to Health Canada in support of Vanessa’s Law.
**Quality, Safety and Risk Management**
- Provides support on an organizational basis through consultation and education on risk reduction strategies.
- Utilizes a number of quality and risk tools, lean methodologies to ensure quality improvement and risk management best practices are being followed at all times
- Assists in the timely investigation of select safety events through the implementation of cause analysis and use of other investigative tools.
- Supports interdisciplinary quality of care reviews (IDQR), and incident analysis.
- Maintains accurate records of recommendations and actions required as a result of incident analysis.
- Establishes and maintains effective relationships with staff, managers and physicians in order to influence and advance safety practices and behaviors.
- Attends working groups/committees throughout the organization to provide a safety and risk perspective.
- Serves as a resource to managers on effective corrective action design to minimize event recurrence.
- Reviews coroner's inquest reports with the appropriate areas of the organization to assess the applicability of the recommendations at Sinai Health.
- Understands and applies the privacy framework within a hospital setting, including assisting with release of information requests.
- Performs other position related duties as required.
- Job Requirements- Undergraduate degree in a related healthcare discipline from a recognized educational institution.
- Member of a health discipline in good standing.
- Minimum 5 years’ hands-on clinical experience with the ability to understand and navigate the patient care process.
- Applicants with proven equivalent recent and related training and experience may be considered.
- Recent and related experience in a safety role within a hospital setting a definite asset.
- Patient Safety / Risk certification(s) preferred.
- Process improvement and project management experience preferred.
- Advanced computer skills; proficient using Microsoft Office (e.g. Outlook, Word, Excel, PowerPoint).
- Strong interpersonal and communication skills; recognized for ability to demonstrate extreme discretion and diplomacy when handling confidential matters and materials.
- Strong relationship-building, facilitation, leadership and negotiation skills.
- Excellent problem solving, decision-making skills with the ability to generate ideas and a fresh perspective.
- Ability to review and analyze data and ability to generate metrics as well as written and verbal reports.
