The Role

Working within a multidisciplinary team, the Mental Health and Addictions Navigator is responsible for providing comprehensive case management services, ensuring that clients receive the necessary care and support to address their mental health and addiction needs. They will collaborate with clients to develop personalized care plans, they will facilitate access to various services, such as counseling, therapy, and social support, tailored to meet each client’s unique requirement. They will work closely with other healthcare professionals, community organizations, and agencies to ensure seamless and coordinated care for clients. Ultimately, this position will aid in Community Wellbeing staff’s ability to connect client appropriate Mental Health and Addictions services and supports that are client /family centered, holistic in nature, focused on prevention, promotion and early intervention, and firmly rooted in Métis traditions and cultural values within their respective communities.
This position reports directly to the Supervisor of Mental Health and Addictions.
In carrying out the following duties, it is expected that the Mental Health and Addictions Navigator will:

- Fully understand the significance and fundamental importance of MNO’s Statement of Prime Purpose (SPP) to all MNO’s work, and to the requirements of the position
- Adhere to MNO Policies and Procedures and MNO’s Financial Policies and Procedures at all times
- Uphold MNO’s Standards of Professional Conduct at all times
- Adhere to Privacy Laws and respect confidential information at all times
- The employee must be cognizant of the Ontario Employment Standards Act, Occupational Health and Safety Act, Workers’ Safety and Insurance Board, Ontario Human Rights Code, Canadian Human Rights Act, and other legislative provisions governing the employment relationship between MNO and the employee
- Ensure that all activities are conducted in accordance with established Métis Nation of Ontario Policies and Procedures and Contribution Agreements (CA)
- Work in collaboration within the Branch, other MNO branches, MNO leadership, MNO citizens, MNO contractors and consultants on related projects
- Attend the Annual General Assembly (MNO Policy 2.120) and MNO public service training as directed.

The incumbent is responsible for the following items, as well as other duties and responsibilities as may be assigned by the Designated Approving Authority:

- Assessment: Conduct comprehensive assessments of clients’ mental health and addiction needs, considering their unique circumstances, strengths, and challenges.
- Care Planning: Collaborate with clients to develop personalized care plans that address their mental health and addiction goals. Ensure that plans are client-centered and aligned with evidence-based practices.
- Service Coordination: Facilitate access to a range of services, including counseling, therapy, medication management, and social support, to meet clients’ needs.
- Advocacy: Advocate for clients’ rights and needs within the healthcare system, ensuring they receive quality care and support.
- Assessment: Conduct comprehensive assessments of clients’ mental health and addiction needs, considering their unique circumstances, strengths, and challenges.
- Collaboration: Work closely with other healthcare professionals, community organizations, and agencies to ensure seamless and coordinated care for clients.
