Job DetailsDescriptionThe Case Worker plays a key role in supporting the East Region's Regional Coordinated Access model by providing client-centered intake, screening, navigation, and service coordination across the mental health and addictions (MHA) system. This role facilitates smooth access to care by reviewing referrals, engaging clients and families, conducting brief screening, and linking clients to appropriate services. The Case Worker also contributes to process improvement, stakeholder engagement, and data collection to ensure the Provincial Coordinated Access model is implemented with fidelity, efficiency, and equity.*Key ResponsibilitiesClient Intake, Screening & Navigation*Review incoming referrals and determine eligibility for MHA services, using standardized screening tools and protocolsConduct brief triage interviews to understand client needs and match them to appropriate programs using a stepped-care approachSupport clients in completing pre-screening measures, consent forms, and enrollment into patient portalsRedirect referrals outside eligibility criteria to appropriate resources (e.g., based on geography or age)Maintains meeting established volume targetsCare Coordination & System NavigationProvide practical assistance related to social determinants of health (housing, finances, education, employment) to help remove barriers to careFacilitate communication with primary care providers, referral sources, and partner agencies to ensure continuity of care and smooth transitionsConnect clients and family members to waitlist supports (self-help, online tools), and crisis resources as appropriatePartnership Development & Stakeholder EngagementFoster positive relationships with referral sources, community partners, and service providers across the East RegionParticipate in team meetings, regional knowledge exchange sessions, and quality improvement activities to enhance service delivery and coordinated accessData, Reporting & Program SupportAccurately document all screening, triage, and navigation activities within the coordinated access technology systemsCollect data to support performance measurement, evaluation, and continuous quality improvement initiativesParticipate in research and program development initiatives that strengthen coordinated access and client outcomesHealth & SafetyWork in compliance with all health and safety regulations, including organizational standards and policiesRecognize and report client, staff, and environmental safety issues and participate in implementing corrective actions*Qualifications*Education: College diploma or Bachelor's degree in Nursing, Social Work, Psychology, Health Sciences, Behavioural Sciences, or related fieldExperience: Minimum 3–5 years' experience in mental health and/or addictions sector, including intake, navigation, or case managementKnowledge & Skills:Familiarity with stepped-care and coordinated access models.Knowledge of mental health and substance use conditions, evidence-based interventions, and community resources.Strong assessment, triage, and service planning skills.Excellent interpersonal, communication, and documentation abilities.Ability to work independently and collaboratively within a multidisciplinary and virtual team environment.Proficiency in electronic health record systems and Microsoft Office SuiteAbility to manage competing priorities and work under pressureOther Requirements: CPR/BLS, valid driver's license, and ability to travel within the region.Freedom of ActionWorks independently and interdependently within a multidisciplinary team, following program standards and organizational policiesEscalates complex cases and safety concerns to the Clinical Manager as neededWork is monitored through documentation review, regular supervision, and performance discussionsLink to OPSEU Wage Table-This position is part of the OPSEU bargaining unitVacancy Available 7Job Posting Closing: November 24th at midnight.
