Roles and Responsibilities
The program director oversees the development and implementation of all Sanctum Care Group (SCG) programs. The program director ensures quality and performance related measures are achieved in relation to program delivery and oversees the medical and social components of care for all clients served by Sanctum Care Group. The program director assists with recruitment, training and support to staff in the delivery of care to patients. The program director is responsible for leading the organization in best practice as it relates to harm reduction, trauma informed care, HIV prevention and treatment. The Program Director is responsible for overseeing a integrated team of professionals both within a inpatient care setting and in the community. The Program Director will ensure strong partnerships with health, social service and non-for-profit organizations in order to coordinate care & supports. The Program Director will be committed to the organization’s trauma informed approach to care and harm reduction principles, ensuring all interactions are dignified, non judgemental, unconditional, and provided without prejudice, living the mission and values of Sanctum Care Group.
Qualifications:
- Registered nurse or Nurse Practitioner
- 5- 10 years experience working in an acute care setting or equivalent care for complex patient populations
- Experience in a leadership role would be an asset
- Expert understanding of trauma informed care and principles of harm reduction and demonstrated ability to implement in practice.
- An understanding of the issues related to HIV and other communicable diseases, poverty, addiction, trauma and mental health.
- Strong decision-making skills with the ability to work confidently and independently to resolve problems.
- Excellent time management skills and the ability to thrive in a fast-paced environment with competing priorities.
- Strong written and verbal communication skills.
**Responsibilities**:
- Provide continuous training and coaching to ensure staff have appropriate level of education, training and understanding to fulfill the vision, mission and goals of SCG
- Identify and work with the Operations Director to respond to deficits in staff and develop a plan to address issues or deficits that arise as it pertains to the day-to-day operations of SCG programs, including disciplinary action when necessary.
- Oversee the clinical care being provided by the Registered Nurses within SCG
- Check in with all SCG’s teams weekly to ensure care provisions are being met as per the goals of each program.
- Ensure appropriate policy and procedures are reflected in the various programs including staff safety.
- Ensure confidential information is handled according to HIPA.
- Review Progress Notes and address any communication deficits.
- Regular audits of Program Metrics to ensure collection is being facilitated correctly and in a timely fashion.
- Coordination of medical and clinical staff to ensure the care of clients is being facilitated and monitored
- Monthly MAR & PRN Medication reconciliation
- Round with pediatrician and NP when required.
- Manage Program Wait lists & program occupancy levels.
- Ensuring appropriate flow of information within SCG structure.
- Devise evaluation strategies to monitor performance and determine the need for improvements in care provision
Pay: $120,000.00-$140,000.00 per year
