**Summary**:
Native Child and Family Services of Toronto strives to provide a life of quality, well-being, caring and healing for our children and families in the Toronto Aboriginal Community. We do this by creating a service model that is culture-based and respects the values of Aboriginal people, the extended family, and the right to self-determination and through the integration of holistic, culturally based prevention services that are child centred, family focused, and community driven.
The Holistic Intake Assessment Worker links Aboriginal children, youth and families to services and support within Holistic Services to increase their well-being spiritually, mentally, psychologically and physically.
**Primary Responsibilities**:
Reporting to the Intake Team Supervisor, the Holistic Intake Assessment Worker is responsible for:
- Supporting community members in their healing journey at the intake phase through the assessment of their needs and referrals to appropriate supportive service programs.
- Creating, processing, and directing intake referrals to appropriate internal service providers.
- Providing brief service support to community members, as required, and connecting with external crisis services as necessary.
- Assessing referrals for: accurate information, eligibility for requested services, important contextual information, and best place for file assignments
- Conduct client intake by performing duties such as obtaining demographic information, providing information regarding services offered through programs, assisting clients in completing intake forms, assessing needs, level of risk, and suitability for the program, and explaining issues related to confidentiality
- Screening referrals that are not filled out correctly and following up with the referring worker as needed
- Suggesting better program fit as appropriate
- Advocating on behalf of the client in navigating the social services and health care systems and ensuring the accessibility and availability of services.
- Promoting and enhancing the services of the program and the image of the agency as a representative with external agencies and other professionals.
- Creating program service files and maintaining required records in accordance with established procedures.
- Provide support and consultation to colleagues, identifying gaps in service and recommending solutions.
- Participating in meetings, planning and evaluation initiatives, committees, events and activities.
- Orienting new staff and providing other related assistance as required.
- Ensuring confidentiality and maintaining and updating files in accordance with NCFST policies and/or legislative requirements in Ontario.
- All other duties related to the programs mandate as assigned.
- Work in a safe manner in accordance with the Society’s Health and Safety Policies and Procedures and all relevant legislation
**Qualifications, Knowledge and Skills**:
- Comfortable with Caseworks system
- Comfortable learning new technology systems and implementing data management processes
- Excellent documentation, assessment, and problem solving skills
- Comfortable consulting with and supporting staff
- Strong knowledge of NCFST services, especially holistic services
- Able to sort through competing priorities
- Able to work independently
- Strong understanding and practice of Indigenous approaches to healing, including land-based practices and cultural knowledge
- Connection to FNIM communities and knowledge and awareness of the diverse urban Indigenous population
- Desire to learn clinical skills combined with Indigenous traditional approach
- Pass a vulnerable sector police records check
- Understanding of client-centred and First Nation, Metis and Inuit community-based work
- Team skills to work collaboratively with others
- Effective counselling and crisis support skills
- Excellent communication skills, both oral and written
- Time management, organizational and coordination skills
- Understanding of Native Child and Family Services of Toronto Mission and values
