As a valued member of the management team, the Resident & Family Experience Coordinator demonstrates a commitment to resident safety by providing quality services in accordance with the organization’s vision, mission, and values.

The Resident & Family Experience Coordinator will be responsible for the coordination of care and services within an assigned resident care home area(s)/neighbourhood(s) to provide exemplary customer service for the resident/family and team member. This leadership approach leverages the skills and expertise of all team members to work together to provide shared leadership, problem solving, and resource allocation to support the care and delivery of optimal resident quality of care.

**As a Resident & Family Experience Coordinator your typical day will impact in the following ways**:

- Management of the Resident Home Area/Neighborhood
- Reports to the Director of Care
- Coordinates daily care needs within the assigned resident home area(s)/neighborhood(s), interacting with the inter-professional team to prioritize resident care activities
- Works in collaboration with other team members, such as Associate Director of Care, Registered Practical Nurses, Registered Nurses, and allied health team members to deliver resident care
- Coordinates resident move in and move out process
- Follows up on concerns/complaints that arise in the resident home area(s)/neighborhood(s) to ensure that they are addressed in a timely manner and achieve satisfactory outcomes
- Assists with the recruitment and retention of team members
- Coordinates the orientation of new team members
- Collaborates with Human Resources to support an Early & Safe Return to Work Program
- Coordinates, attends, and participates in resident care conferences
- Assists in conducting investigations
- Ensures supplies management and resource allocation including the ordering of resident care supplies, continence supplies
- Conducts assigned Quality Management Activities as per schedule to improve resident care and services
- Ensures Infection Prevention & Control measures are in place to promote resident and team member health and wellbeing
- Participates in the development of quality improvement plans to address identified areas of risk and opportunities for improvement
- Acts on resident/family survey results to improve satisfaction with care delivery
- Conducts in-service sessions to address topics related to care and services
- Supports the team in management of resident risk and implement de-escalation measures to promote safety for risks/team members and visitors
- Works in accordance with the organization’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act
- Performs other duties as assigned

**Must haves**:

- College Diploma or University degree or equivalent is preferred
- Minimum of three years’ experience in healthcare, social services, or related sector
- Experience in the field of Gerontology preferred
- Knowledge of the Long Term Care Act & Regulations preferred
- All applicants must successfully pass the prescribed Vulnerable Sector Check. Applicable to Ontario applicants.

**Job Types**: Full-time, Permanent

**Salary**: $40,000.00-$50,000.00 per year
