Resident & Family Experience Coordinator (Social Services Worker) Join us as a Resident & Family Experience Coordinator II, where you will play a critical role in improving the lives of residents and fostering a welcoming community. You will lead the move‑in, transfer, and move‑out processes, provide social‑services counseling, and coordinate advanced care planning and treatment plan discussions. Responsibilities Lead the resident move‑in, transfer, and move‑out process in accordance with the organization’s move‑in experience standards, including facilitating the application, accessing financial supports, tours, and introducing the resident/family/SDM to the community and its operations. Provide social‑services worker counseling services within the scope of qualification. Plan, develop, implement, and evaluate individual social work services and/or support groups to meet the identified needs of residents and families. Engage external resources when required to ensure resident social needs are met. Support residents and families during advanced care planning, treatment plan conversations, and funeral arrangement discussions. Respond to, correspond with, and track feedback received in the resident home area/neighborhood, ensuring timely and satisfactory resolution. Provide reporting and analysis of feedback to Leadership & Quality Committee. Collaborate with departmental leaders to coach and direct resident home area/neighborhood teams, delivering exemplary service excellence and hospitality. Qualifications Registered Social Services Worker per provincial legislation. Minimum three years’ experience in healthcare, social services, or related sector. Knowledge of the Long Term Care Act & Regulations preferred. Customer service related experience is an asset. Hospitality and/or sales related experience is an asset. Strong oral and written communication skills required. Ability to organize, plan, and delegate responsibilities.
