**Position Description**:

Reporting to the Manager of the Brant Assertive, Community Treatment Team (Brant ACTT), the registered Social Worker will utilize a full scope providing consultation to interprofessional team members and clinical case management to individuals diagnosed with serious and persistent mental illness. The Social Worker plans, implements and evaluates psychiatric assessments and evidence-based treatment; including time limited individual and group treatment. They will promote a psychosocial perspective congruent with social work training.
As a member of an interdisciplinary service, the Social Worker will participate in the formulation of individual and group treatment programs, collaborate with other interprofessional staff and community partners to develop, implement and evaluate person/family centered programming, and maintain Standards of Practice as defined by the College of Social Work. The Registered Social Worker provides leadership in high risk social situations, program development, advocacy, and developing and maintaining strong community linkages. Registered Social Workers will also participate in mentoring, community development, teaching, continuous quality improvement and research activities.

**QUALIFICATIONS**
- Clinical MSW and/or BSW from an accredited university. MSW is preferred
- Current registration with the Ontario College of Social Workers and Social Services Workers required
- Membership with the Ontario Association of Social Workers preferred
- Minimum of two-five years’ experience in an outpatient mental health setting
- Demonstrated knowledge of clinical psychiatry and psychotropic medications
- Demonstrated current knowledge of the signs and symptoms of mental illness, social work intervention strategies, and treatment modalities for clients with psychiatric disorders
- Demonstrated crisis intervention, case management skills and knowledge of behavioral technique
- Demonstrated knowledge of Mental Status Examinations (MSE) including current knowledge of the signs and symptoms of mental illness, social work intervention strategies, and treatment modalities for clients with psychiatric disorders
- Demonstrated knowledge, training and recent experience in providing individual and group based Dialectical Behavioural Therapy (DBT) in an outpatient setting
- Demonstrated knowledge, training and recent experience in delivering individual and group based Cognitive Behavioural Therapy (CBT) in an outpatient setting
- Demonstrated advanced knowledge of individual counseling and group facilitation skills specifically related Cognitive Processing Therapy (CPT) in an outpatient sitting preferred
- Demonstrated knowledge of individual CAMS counselling would be an asset
- Demonstrated knowledge of individual counselling and group facilitating of (CPT, PE, CBT, MI) preferred
- Demonstrate ability to develop and maintain effective professional and therapeutic relationships with clients
- Demonstrated knowledge of relevant legislation which includes: Mental Health Act, Healthcare Consent Act, and Substitute Decisions Act
- Demonstrated capacity to work autonomously as well as in collaboration within an interprofessional healthcare team
- Demonstrated ability to work as an effective member of an inter-professional team with exceptional interpersonal, advocacy, negotiation and conflict resolution skills.
- Exceptional, communication; verbal and written skills, interpersonal, critical thinking and creative problem-solving skills
- Ability to work effectively to ensure patient flow (admission, discharge processes) and effective caseload management within established targets
- Excellent organizational and time management skills, including ability to prioritize workload
- Willingness and ability to continue to improve professional competence and knowledge
- Demonstrated ability to work under pressure, prioritize workload, and meet deadlines
- Proficient computer skills for report writing, assessments (i.e., OCAN), word processing, workload measurement (ATR), and access to information
- Ability to go work within the community to support patient care and transition to other community programming, including working at other SJHH sites and alongside other programs
