**Job Overview**

**Duties**
- Conduct comprehensive assessments of patients' social, emotional, and environmental needs.
- Develop and implement individualized treatment plans in collaboration with healthcare teams.
- Provide case management services to ensure continuity of care for patients.
- Facilitate communication between patients, families, and medical staff to promote understanding of treatment options.
- Offer counselling and support to patients/clients and their families during difficult times.
- Maintain accurate records of patient interactions and interventions in compliance with The Couch Counselling policies.

**Skills**
- Strong research skills to identify resources and best practices in social work.
- Experience in case management.
- Ability to provide compassionate patient/client care while maintaining professional boundaries.
- Proficiency in assessment methods for evaluating patient progress and program effectiveness.
- Excellent communication skills for effective collaboration with interdisciplinary teams.
- Empathy and patience when working with children and their families facing challenging situations.

**Job Types**: Freelance, Internship / Co-op

Pay: $60.00-$65.00 per hour

Expected hours: 10 - 15 per week
