**Position Summary**

The Director, Quality, Patient Safety, Risk, Integrated Policy & Interprofessional Practice will develop, execute, and evaluate an integrated quality improvement, patient safety, interprofessional practice, policy and risk management strategy and structure at OSMH.

The position will build organizational capacity to effectively identify and address quality and safety issues hospital wide while leading and supporting process changes to enable OSMH to spread the capability and capacity for quality improvement across the hospital. The Director will provide high-level strategic and operational guidance to position OSMH as a leader in quality improvement and contribute to the achievement of OSMH’s strategic priorities and goals.

The position will provide internal expertise, leadership, and oversight for leading the planning, development and execution of functions in the areas of Quality, Risk Management, Ethics, Patient Safety, Interprofessional Practice and, Integrated Policy. The Director has corporate responsibility for oversight of the Hospital Accreditation process.

The Director works collaboratively with the management team, program and service teams, credentialed staff, team members and external stakeholders to support continuous performance improvement that will positively impact the patient and family experience. This role will foster strong relationships and close collaboration among all levels and areas of OSMH including physician leadership to successfully implement quality improvement priorities for organization wide transformation of quality improvement.

The Director ensures that safety, learning, and accountability systems are used and applied consistent with a just culture, and that leaders are closing the loop on inspections/audits, incidents, risk assessments and other staff and patient safety issues, to ensure continuous quality improvement.

Quality & Risk

The Director will develop, implement, monitor, evaluate and continuously improve upon a comprehensive and integrated organizational patient safety, quality improvement and risk management program. The work of the Director will be aligned with provincial and national directions in quality improvement, risk management and patient safety. This mandate is to be achieved through learning from patient experiences how to improve care, through planning, development, implementation, integration, and evaluation of strategies to build a culture of safety and quality. The Director is also responsible for responding to clinical risk situations through tracking and learning from patient safety events, conducting structured quality reviews, as well as supporting mitigation strategies/ recommendations. Quality is fundamental to the hospital and the oversight of ensuring the QI framework is utilized organization wide and embedded as part of the culture is key.

Interprofessional Practice

The Director provides leadership to advance the practice of clinical professionals providing care. The Director is responsible for ensuring best practices in the organization by leading the development, maintenance and evaluation of clinical practice systems and processes to advance patient care planning and documentation. This mandate is to be achieved through assessment of learning needs and addressing learning needs through planning and deployment of professional education resources in consultation with Program Directors. Leadership for interprofessional practice standards is to be achieved through literature review, development of practice tools (policy, procedure, order sets, care pathways) and support for implementation.

Patient Safety

The Director has accountability for the development and implementation of an inclusive patient experience plan that seeks to enhance the experience through continuous quality improvement. Capitalizing on the data from the Incident Management System and other mechanisms for feedback, provide robust reporting on safety issues and mitigation strategies.
