**Description**

The Case Manager is central in the coordination of services offered through CBIS as well as referrals to other important community services. They are responsible for conducting intakes, establishing rehabilitation plans and assisting clients to meet goals, and supporting clients over time to assist in navigating the ever-changing landscape of living with brain injury. The Case Manager nurtures connections with community organizations and resources and utilizes those resources to be able to provide survivors and their families with a broader range of services and supports than would be possible through CBIS alone.

**Job Requirements** 
- Diploma or degree in Health or Human Services
- Experience working with a trauma-informed approach
- Training or experience in Mental Health
- Familiarity, sensitivity and respect for cultural needs of the Indigenous/First Nations community
- A vehicle, and valid BC Driver’s License 
- Safety oriented first aid & CPR certificate 
- Minimum of 3 years human service experience 
- Minimum of 1-year experience working with survivors of brain injury 
- Equivalent combination of education, training, and experience

**Hours **
35 hours/week. Monday to Friday, occasional evening or weekend attendance at events.

**Pay Rate**
$28.50/hour to start. $30/hour following a successful 3-month probation.

**Location** 

 CBIS office at 481 Trans Canada Hwy with the ability to at times work from home 1-2 days per week.

**Job Types**: Full-time, Permanent

Pay: From $28.50 per hour

Expected hours: 35 per week

Schedule:

- Monday to Friday
- Weekends as needed
