About the Role:
Are you ready to make a positive impact and be an active force in the community? We are currently seeking a dynamic Community Disability Worker to join our dedicated team. In this community-based role, you will have the exciting opportunity to **establish and maintain meaningful connections** with Supported Individuals in various community settings. You'll actively engage in events, appointments, and activities while **providing essential support and mentorship.**

To excel in this role, you must **maintain strong boundaries**, practice good self-care, and possess the ability to make a real difference in the lives of Supported Individuals.

As a mentor and role model, you will inspire Supported Individuals to achieve their goals and **unlock their full potential. **Your youthful energy and fresh perspective will help create a dynamic and engaging environment.

Responsibilities: As a Community Disability Worker, you will have the opportunity to make a real difference in the lives of Supported Individuals. Your key responsibilities will include:

- Act as a mentor and a role model for Supported Individuals.
- Maintaining accurate program administration records, such as monthly summaries, contact notes, and Supported Individual schedules, following our established agency procedures.
- Proactively addressing Supported Individual goal areas and tailoring teaching approaches to their specific needs, whether as a leader, coach, hands-on supporter, or mentor.
- Exhibiting flexibility, independence, and excellent communication skills to foster a positive and empowering environment.
- Identifying and documenting the strengths, limitations, and behaviors of Supported Individuals in line with program procedures.
- Collaborating effectively with internal and external teams to ensure comprehensive support.
- Modeling a respectful attitude towards Supported Individuals and fellow team members.
- Demonstrating a commitment to personal development by following directions, setting personal goals, and seeking support and guidance.
- A positive attitude and belief in the rights and responsibilities of persons with disabilities to define and pursue their life choices while actively participating in the community.
- A comprehensive understanding of the opportunities and options available to individuals with disabilities.
- A high school diploma, along with a minimum of one year of relevant additional training from a recognized institution, complemented by at least 1+ year(s) of progressive direct service experience.
- A demonstrated ability to work effectively in a collaborative, team-oriented environment.
- Strong boundary-setting skills, excellent communication abilities, and a passion for promoting mental health and well-being.
- Proficiency in computer skills and report writing, aligned with the specific role and agency requirements, including basic internet search skills.
- Current First Aid and CPR certification, Abuse Protocol Training, Medication Administration Certification, and Mandt training.
- Knowledge of personal health and safety guidelines and regulations.
- A well-developed understanding of the vast array of community resources available to support the participation and inclusion of Supported Individuals, with the ability to build and maintain a strong community network and act as an advocate for the profession.
- Full COVID-19 vaccination is a condition of employment.
- Possession of a reliable car with personal liability insurance, providing a minimum coverage of two million ($2,000,000.00).
- Possession of a Class 5 - Non GDL Drivers License with a Clean Drivers Abstract and no restrictions.
- Current (within the last 6 months) Criminal Record Check with a Vulnerable Sectors search.
