Community Disability Worker

**Overview**
The Community Disability Worker (CDW) provides direct support to Individuals Accessing Services (IASs) in achieving their goals and leading fulfilling lives. This frontline position supports adults or youth with developmental and physical disabilities. This includes providing medical, technological, environmental, emotional, physical, and behavioural support to IASs. The CDW helps individuals reach their goals by actively engaging with them, learning their interests, and supporting them to succeed.
This position participates in the research, development, and implementation of programs to meet IAS needs and proactively identifies and develops solutions for IAS concerns. The CDW also provides input in the development of IAS support plans (ISPs, PPP&RPs) and is responsible for updating and maintaining these documents and other required IAS documentation. This position is responsible for ensuring that all IAS documentation is completed in an effective and timely manner using the 4Cs: clearly, concisely, coherently, and considerately.
This position strives to create a culture of high performance and teamwork by consistently demonstrating excellent communication, problem-solving, and decision-making skills. The CDW must follow all Vantage policies and procedures, health and safety protocols, legislative protocols, and strive to meet CET standards at all times.
**Accountabilities**
**1. Ensure that the day-to-day needs of IASs are met, and that Vantage policies and procedures are being consistently followed**
- Proactively build positive relationships with IASs
- Provide medical, technological, environmental, emotional, physical and behavioural support to IASs
- Advocate for IASs in the community and ensure that IASs are being supported and actively integrated in the community
- Ensure to safeguard IASs’ rights/values and empower them to make their own decisions
- Be organized and proactive in the scheduling of IAS’ daily activities
- Administer medication correctly for each IAS and follow appropriate policies and procedures
- Submit verbal and written reports of any accidents, incidents, or illnesses to the Team Leader immediately andguardian and worker as required
- Assist in coordinating or providing transportation for IAS needs
- Ensure that IASs are taken to medical appointments if needed
- Ensure that all health and safety protocols are followed consistently
- Ensure that program administrative requirements are consistently and effectively maintained (daily journals, outcome tracking, health summaries, IAS schedules, etc.) in accordance with agency procedures
- Ensure that all IAS documentation is completed in an effective and timely manner using the 4Cs: clearly, concisely, coherently, and considerately
- Ensure all administration and program activities and requirements are met in appropriate timelines as set out by internal and external contacts
**2. Participate in the development and implementation of IAS support plans and achievement of IAS goals**
- Participate in the research, development, and implementation of programs and solutions to meet IAS goals
- Provide input in the development and updating of Individual Support Plans (ISPs), Planned Positive Practices (PPPs) and Restrictive Procedures (RPs) if needed
- Liaise with the Team Leader to complete and update support plans and ensure that goal tracking is completed throughout the year (outcome tracking changes to IAS information, and all relevant information is up to date)
- Participate in IAS network and team meetings as required
**3. Be a positive and contributing member of the team**
- Help create a culture of recognition and appreciation on the team
- Actively participate and contribute new ideas in team meetings
- Consistently arrive for shifts on time and notify the Peer Leader or Team Leader immediately if an unexpected situation arises that causes you to be late
