The Service Coordinator works to coordinate the scheduling of staff with families for the purpose of Community Respite, and private Home Care support.

**WHAT THE ROLE PROVIDES YOU**:

**KEY RESPONSIBILITIES**:

- Provide leadership by coaching and communicating with families, clients and internal staff and stakeholders on a daily or as-needed basis; provide information to support their ability to meet their needs effectively and promptly
- Ensure honest, open, timely and accurate communication at all times to ensure all team members are kept up to date with relevant information
- Provide exceptional customer service and demonstrate professionalism with clients, DDRC staff and external agencies, partners, and businesses Assist in the implementation and integration of administrative systems and processes to support the Comprehensive Care team in an effective manner
- Coordinate with staff, clients and family to ensure client support is scheduled in a timely and professional manner in accordance with requests, skill sets and competencies Monitor and adjust schedules on a daily basis to ensure compliance with OHS and for leave of absences
- Maintain accurate, up to date and complete records for employees, clients, and family
- Must remain knowledgeable of all community resources and referral options
- Communicate and schedule mandatory training requirements of employees as needed Complete all paperwork accurately and on time
- Highly knowledgeable of all DDRC Home Based Services programs and services
- Review and track timesheets, mileage and other expense claims to client services delivered and submit reviewed documents to Team Leader for approval
- Prepare weekly error-visit documentation
- Perform all other duties assigned by the Team Leader, Comprehensive Care

**WHAT YOU BRING TO THE ROLE**:

- Minimum 2 years post-secondary education and or a combination of 1 year post-secondary education and experience
- Takes accountability to ensure all mandatory certifications and training are kept current
- Attend all required meetings and planning sessions as requested; accountable for seeking information if absent from meeting
- Participate in agency promotion events, client and family service fairs and in transitioning seminars from children to adulthood
- Ability to remain calm and composed in stressful situations
- Accuracy and attention to detail while working under tight deadlines
- Demonstrated proficiency in MS Office, with particular emphasis on Microsoft Excel, Word and PowerPoint
- Knowledge of and understanding of applicable federal and provincial legislation as well as professional standards (e.g. Freedom of Information and Protection of Privacy Act, Protection of Persons in Care Act, Core Standards)
- Creating Excellence Together (CET) Accreditation experience within the disability sector, an asset
