**Harbourview Family Health Team (HFHT) **provides primary health care, including disease management and prevention, health promotion, health assessment and therapeutic interventions to a rostered population of patients in the city of Thunder Bay. The FHT employs a multidisciplinary team of health care providers who work in a collaborative model of care.

The Mental Health Counsellor, as part of the interdisciplinary team, assists in the primary care functions and participates in the development, implementation, monitoring and evaluation of mental health programs and services. The Mental Health Counsellor provides counseling/therapy and other therapeutic interventions for individuals.
**ACCOUNTABILITY**
The Mental Health Counsellor is to practice within the scope and according to the standards of practice as outlined by the Ontario College of Social Workers and Social Service Workers and the Regulated Health Professions Act. The Mental Health Counsellor reports to and is accountable to the Executive Director of the Family Health Team.
**DUTIES AND RESPOSIBILITIES**
- In collaboration with Physicians and other Allied Health implement and discuss appropriate individualized care plan with the clients based on best practices.
- Facilitate linkages with appropriate services, supports, and resources within the community.
- Assist clients to set goals and evaluate achievement of client goals
- Documents using client health records, refers to and arranges follow-up as necessary.
- Prepare monthly or as required statistical reports for the Ministry of Health and the Harbourview Family Health Team.
**Education, Qualifications and Skills and Experience**
- Graduate degree in Social Work, preferably Masters or working towards.
- Three to five years counseling/therapy experience: preferable in a community setting
- Proficiencies in assessment, psychotherapy, and psychological counseling techniques for working with individuals.
- Registration with a governing College such as the ocswssw.
- Comprehensive knowledge of local and virtual resources and services
- Experience in program development, implementation, monitoring and evaluation
- Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction.
- Superior leadership, organization, research, evaluation, time management, communication, and interpersonal skills.
- Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel, Outlook.
- Prior experience and knowledge of electronic medical record.
- Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences.
- Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient.
- Exhibit an ability to be open and non-judgmental.
- Knowledge of the goals and structure of the organization, the policies and procedures and the programs.
- Experience with Oscar WELL EMR, MS outlook, Oceans.

- Communication - Excellent verbal and written communication skills, writes clearly and concisely; Able to read and interpret written information.
