**WHAT YOU WILL DO**

Reporting to the Managers of Clinical Service, the Intake Coordinator (IC) is responsible for the assessment of client statuses and facilitating intake, client education, and orientation for new clients. This role oversees client case management and the respective administrative duties.

You’ll collaborate closely with colleagues, as well as third-party funders. You’ll work in a fast-paced environment as an integral member of a dynamic multidisciplinary team, while sharing our core values of communication, community, excellence, health, and innovation.

**Comprehensive client-centered care**
- Effective assessment of client status
- Facilitate client intake appointments, tours, and client education
- Ensure smooth transition of client from intake phase, through treatment, and into discharge
- Provide comprehensive case management and client support
- Support clients receiving substance abuse treatment with biological monitoring, relapse prevention planning, motivational interviewing
- Respond to inquiries from prospective clients, as needed.

**Team Collaboration**Accountabilities**
- Facilitate weekly case consultations and complex case discussions with the interdisciplinary team
- Communicate team decisions, care needs, and actionable items from weekly case consultation to appropriate staff and or clients
- Communicate with other clinicians on a regular basis about issues related to client care

**Administrative**Accountabilities**
- Ensure same-day completion of session notes
- Complete assessment and discharge reports in a timely manner
- Manage incoming referrals from stakeholders, and community partners
- Provide reporting to third parties, community referral sources, and stakeholders, in a timely and professional manner
- Monitor schedule and client progress to ensure clients are booked in all necessary and appropriate groups/appointments
- Collaborate with administrative staff to optimize clinician schedules in order to minimize wait times and ensure rapid access to programs.

Other responsibilities related to the role and scope of work may be assigned.

**WHAT YOU BRING**

**Experience**
- A minimum of two years of experience supporting clients with mental health conditions, including assessment/triage and treatment, is required.
- Experience with client coordination and/or case management is an asset.
- Customer service experience is an asset.

**Education**
- Bachelor's degree, ideally in Psychology, Social Work, Nursing or a related field.
- Equivalencies of education may be considered.

**Technical Competences**

Proficiency in the Microsoft Office Suite, including Word, Excel, PowerPoint, Outlook, and Teams is required. Proficiency with Electronic Medical Records (EMR) systems is also an asset.
