**Overview** Open Arms Resource Center is committed to transforming lives by providing compassionate, impactful services to individuals experiencing homelessness. Open Arms is seeking a motivated and compassionate Outreach Case Worker to join our growing team. This one-year role is renewable pending funding. In this role, you will be responsible for building relationships with individuals experiencing homelessness through outreach and engagement, conducting intake assessments, and developing individualized case management plans to address complex challenges. You will also maintain ongoing support in the community through meetings and accompaniments, coordinate supports and referrals, and advocate on behalf of individuals with health issues, substance use, mental health, and housing concerns. You will provide client - centered care, work to connect clients to housing, healthcare, and support services. Key Responsibilities: Outreach - Building relationships with individuals experiencing homelessness through outreach and engagement. - Conducting intake assessments with participants to identify needs and develop individualized case management plans to include the coordination of supports, referrals, and advocacy on health issues, substance use, mental health, housing etc. - Maintaining ongoing support in the community through meetings and accompaniments. - Working collaboratively with Open Arms Shelter & Housing teams, along with external service providers including healthcare centers, law enforcement agencies etc. - Supporting client engagement initiatives in the community. - Intervening and de-escalating crisis situations. - Maintaining accurate case management records and completing reporting as required. Qualifications: Education: Post-secondary education in a relevant discipline i.e. social work, social or human services, or related field, or an equivalent combination of education and experience. Skills & Experience: - 2+ years’ experience in homeless services, outreach, or case management, or a related field. - Ability to remain calm and effective in high-pressure situations. - Knowledge of local resources and support networks for homelessness and mental health would be considered an asset. - Experience working with highly vulnerable populations, particularly those with health, mental health, and substance abuse issues. - An understanding of the concerns and needs of homeless individuals living in encampments, and the ability to communicate effectively with them. - Excellent communication skills required, including the ability to work in a respectful and cooperative manner with co-workers, clients, and community partners. - Valid certification in Non-Violent Crisis Intervention and First Aid/CPR or willingness to be trained. - Local travel required, including ability to operate the Outreach vehicle; must have a valid driver’s license and access to reliable transportation. - Ability to work with mínimal supervision and the flexibility to solve problems creatively - Culturally competent with a passion for community engagement and social impact. - Proficiency in Microsoft Office Suite; familiarity with CRM systems is an asset.
