The Supportive Living Program (SLP) provides congregate housing and support for youth in Kentville, Windsor and Middleton. A wrap around care team of staff will deliver various support at different levels of care. The

**Wellness Coordinator
- Case Manager**

The SLP Wellness Coordinator will work as a team member, to deliver the Supportive Living Program within one of three home locations. Home locations are in Middleton, Windsor and Kentville, N.S. The homes accommodate as many as 5-9 youth. We provide staff and programming support for gaining life skills, mental wellness and finding belonging in the local community.

**Responsibilities**:
1. Work closely with the SLP Manager to develop and deliver the SLP program.
2. Advise on appropriate programming.
3. Individual case management (up to 10 youth per week) a. Provide 1:1 coaching for clients and facilitate appropriate goal setting. b. Maintaining progress reports in Sales Force or other software (ex: outcome star). c. Collaborate with Life Skills Coordinator on behalf of clients. d. Collaborate with Youth Worker Plus and/or Social Workers when needed. e. Facilitate client conferences with other supporters when needed.

4. Provide intake evaluation of clients seeking admission to SLP.
5. Must have a strong knowledge of mental health and addictions diagnosis and services and work in collaboration with Youth Worker Plus and/or Social Workers.
6. Collect and maintain program evaluations and stats.
7. Support and advocate for clients in navigation of medical, mental health and addiction services.
8. Seek appropriate service providers such as therapists and counselors for clients.
9. Learn about (and document) each client's unique access to services and help them to utilize them effectively.

**Job Type**:Full time salaried based on a 37.5-hour work week.

**Pay Rate**:$48,750 annual + benefits.

**Requirements**:
**Education**:

- Degree in Social Work, Community Development, Sociology or Psychology in combination with experience working with youth at risk (16-24). 4-year degree + 2 years minimum direct experience.
- Or 2 -year diploma in Youth Work, Child and Youth, Addictions and Community Outreach or equivalent training and experience. 2-year diploma + 4 years minimum experience
- Five Core Competencies (MANDT, ASIST, NVCI, Mental Health First Aid and First Aid AED Level C).
- Equity and inclusion training
- Inclusive language training
- Training in Motivational Interviewing

Other training assets: Coaching skills & An understanding of positive youth development

Professional Experience
- Two years minimum experience in case management within the homeless serving sector
