**Duties**
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Monitor patients’ vital signs such as blood pressure, pulse, respiration, and temperature to ensure health stability.
- Administer medications accurately following prescribed care plans while maintaining compliance with HIPAA regulations for patient privacy.
- Support clients with meal preparation and nutritional needs, ensuring dietary restrictions are followed precisely.
- Provide companionship and emotional support to promote mental well-being, especially for those with dementia or Alzheimer’s disease.
- Utilize specialized equipment such as Hoyer lifts or resident lifts to safely transfer clients with mobility challenges.
- Conduct patient observation and report any changes in condition or behavior promptly to healthcare professionals or family members.
- Maintain cleanliness and hygiene by performing light housekeeping duties related to client care environments.
- Follow detailed care plans tailored to each individual’s needs, including assisted living or hospice care requirements.
- Support clients in community settings or within their homes by working closely with families and healthcare teams to ensure comprehensive care delivery.
- Document all care activities accurately while respecting patient confidentiality and HIPAA standards.
- Engage in ongoing training related to CPR, first aid, dementia care, autism support, or other specialized areas as needed for continuous improvement.

**Qualifications**
- Proven experience in home health care, assisted living facilities, nursing homes, group homes, or long-term care environments.
- Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and safety protocols such as CPR and first aid certification.
- Familiarity with working with individuals with developmental disabilities, autism spectrum disorder, dementia (including Alzheimer’s and memory care), or hospice needs.
- Ability to lift heavy objects safely using equipment like Hoyer lifts; physical stamina is essential.
- Excellent communication skills for collaborating with clients’ families and healthcare teams while maintaining confidentiality per HIPAA guidelines.
- Prior experience working in home & community care settings is highly desirable.
- Compassionate attitude combined with patience and adaptability when caring for diverse populations.
- Valid driver’s license may be required depending on the role’s travel expectations; background checks are standard for employment compliance.
