**WHAT YOU WILL DO**:

- **Chronic Disease Management**: Manage and support patients with chronic illnesses, including mental health and addiction concerns, with a focus on evidence-based treatment and prevention.
- **Treatment Planning**: Develop and implement treatment plans, prescribe medications, order diagnostic tests, and interpret results.
- **Mental Health and Trauma Care**: Provide compassionate and empathetic care to individuals dealing with mental health issues, addiction, and trauma, ensuring holistic and patient-centred support.
- **Patient Education**: Deliver health promotion and disease prevention education, with an emphasis on empowering patients to actively manage their well-being.
- **Collaboration**: Collaborate with a multi-disciplinary healthcare team to ensure comprehensive and coordinated care, with a strong focus on addressing the unique healthcare needs of our target populations
- **Referrals**: Facilitate referrals to specialists and community resources as necessary to ensure access to a broad spectrum of care services.

**REQUIRED EXPERIENCE & QUALIFICATIONS**:

- Physician Assistant must be a Canadian Certified Physician Assistant (CAPA).
- Stand First Aid - CPR Level HCP (Healthcare Provider).
- Minimum 1 year experience
- Passion for collaborating with experts from various fields to offer multifaceted care that covers lifestyle medicine, counselling, neurorehabilitation, and an array of services designed to support our unique population.
- Strong interpersonal and communication skills with a patient-centred approach.
