**Role Overview**

The successful applicant can expect to collaborate with the service users, their circle of care, hospitals, retirement/long term care homes, and their team members, etc., accompany service users to appointments, provide reminders to service users, make recommendations within the scope of their role, make referrals to other service providers, and service systems. They will be spending time in the community visiting service users, making calls, collaborating with internal and external agencies and documenting all interactions and information into the service user’s health record. This position will see the service provider visiting service users in various settings, which can sometimes, have odorous smells. The service provider will follow Cota’s policies, guidelines and values when representing the organization.

**About the Team**

The Toronto West Integrated Service Team is comprised of Case Managers who have expertise in a variety of areas including: Acquired Brain Injuries, Mental Health, Geriatric, Dual Diagnosis, and addictions.

**Primary Responsibilities**:

- To work collaboratively with internal and external partner agencies to provide short term CM care for clients 65 years and older with some exceptions.
- To network with external agencies to obtain relevant resources for clients care and to make referrals to Retirement Homes and Long Term Care facilities as if needed.
- To support/encourage clients to participate in community activities to build social network.
- To advocate with and on behalf of clients to improve and/ or maintain wellbeing.
- To create, maintain and improve healthy working relationships with the client and their circle of care.
- To work collaboratively with their team members to support all clients on the TWIST team.
- To attend and engage with others on the Cota Geriatric Community of Practice meetings and network with other Geriatric case managers.
- To be accountable for meeting benchmarks and completing documentations on time.
- To contribute to the overall growth and wellbeing of the Team.
- To keep up with relevant knowledge and information to better support the clients.
- A preference for a Bachelor’s degree in a relevant discipline (i.e. Psychology, Social Work, etc.), AND
- One year of experience in a position working with the Geriatric Population.
- OR, other relevant education and experiences that is determined to be acceptable by the interviewers.
- Knowledge of Dementia and other age
- related illnesses is preferred.
- Knowledge of Diversity Equity and Inclusion issues and Intersectionality.
- Ability to model Cota’s core values with clients, co-workers and partner agencies.
- Knowledge and understanding of the client population and an ability to work effectively with people living with serious mental illness respectfully.
- Demonstrated strong commitment to the rights and ability of each person with a serious mental illness to live successfully in community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services.
- Knowledge of principles of case management theory and practice using a client-centred and mental health Recovery philosophy.
- Excellent clinical judgement and decision-making skills including a keen awareness of service users-clinician boundaries and ethical dilemmas.
- Experience working on a multidisciplinary team is an asset.
- Experience working in a community agency is an asset.
- Experience working with external organizations and building positive working relationship
- PIECES certificate training is an asset.
- Knowledge of Microsoft Office Suite (Word, Excel, etc.).
- Excellent knowledge of community resources and/or demonstrated ability to search for applicable resources/information.

**Working conditions and Other Requirements**:

- Visits will be conducted in the participants’ homes, clinics, hospital, shelters, and other community settings as required.
- Must have access to a vehicle ((Please note that access to a vehicle means owning, borrowing, renting the vehicle. Alternatively, using ride services, such as, Uber, cab, and taxies).
- Availability to work day & evening hours, and weekends as may be required.
- Role adjustment as needed.
