**What You Will Do**
- As a Harm Reduction Case Manager, you will provide in-the-moment, client-centered support and coaching that helps Carepoint and Counterpoint participants living with, impacted by, or at risk of HIV/AIDS, identify and resolve problems in their everyday lives.
- Working within a multi-disciplinary team, you will provide harm reduction & HIV/HCV education (including safe injection and overdose prevention) and distribute harm reduction material to individuals who use substances to support positive health outcomes in your community.
- You will establish rapport with people who use substances and provide practical supports for participants in the community (i.e. facilitating access to transportation, food bank, financial assistance) while documenting these interactions.

**What You Will Bring**
- A strong ability to build rapport with participants and engage with and relate to culturally, linguistically, ethnically, spiritually, racially, and geographically diverse communities.
- A strong knowledge of community resources and social service agencies in London with the ability to identify and understand intersectional factors that impact the health and wellbeing of diverse communities of people.
- Excellent interpersonal and problem-solving skills and strong written and oral communication skills with the ability to deliver effective conflict resolution and crisis intervention techniques.
- The ability to provide support to individuals in the community in an outreach capacity, which could include being offsite in various environments for part or all of your shift.
- First Aid & CPR, BLS, CPI training considered an asset.
- A satisfactory Police Vulnerable Sector Check (VSC).
