Closing the Gap Healthcare (CTG) is dedicated to providing Canadians with the highest level of care across a wide range of home and community settings.

We are seeking a passionate and dedicated Care Team Coordinator to join your team. In this role, you will coordinate and schedule home and community visits, communicate with our healthcare providers, ensuring seamless service delivery while providing empathetic support to the clients and providers. Your administrative skills will help keep our operations organized and efficient.

**What you will be doing?**
- Assist and interact with clients, delivering exceptional customer service and support.
- Presenting information about CTG programs and services to clients to promote service engagement and support sales efforts.
- Schedule visits and manage communication between clients and providers, ensuring timely and effective service
- Accurately and efficiently assessing customer requests and addressing them immediately or escalates as appropriate.
- Keep track of all client interactions, schedules, and relevant documentation to maintain a smooth workflow
- Work closely with an experienced leadership team, benefiting from their guidance and direction as you grow in your role

**Skills needed for success**:

- Exceptional communication and interpersonal skills, with proficiency in English.
- A passion for customer service and a desire to grow within the healthcare industry.
- Commitment to ensuring the confidentiality and privacy of client information at all times
- Organized with precise attention to detail and efficiency in data entry.
- Knowledge of Medical terminology is an asset.
- A diploma from Community College in Communication, Business Administration, Office Administration or Medical.
- Office Administration and/or Grade 12 Diploma with related experience.
