Employment Support Worker

_The Community Disability Services Worker (CDSW) is responsible for front-line work that supports the development of skills and relationships to help adults with disabilities work towards and meet their employment goals. CDSW upholds human rights, health, safety and well-being of all people in service and colleagues._
**Key Responsibilities (include but are not limited to)**:
- Acting as an ambassador of Chrysalis and interacting and marketing Chrysalis and clients with community partners, committees, associations and at company events that may occur outside of regular working hours
- Working alongside the Chrysalis Human Centered Design initiative to ensure that community connections made on behalf of clients are individually based on their unique skills and interests
- Day-to-day tasks ensuring health and safety, personal living skills, assist in personal care, medication, and any other task necessary to support basic needs.
- Engage and participate with individuals and their support network to facilitate and support the documentation of individualized person-centered plans to provide a structure for consistent and on-going support and assistance.
- Facilitate the implementation of individual services plans ensuring collective collaboration with teams, supervisors and support networks.
- Encourage, support and assist individuals to actively exercise their rights and responsibility to make life choices, participate and pursue meaningful, respectful and authentic relationships aligned with self-determined goals and aspiration.
- Know and adhere to established policies, practices, operational protocol, ethical standards and guidelines, medical protocols and standing orders.
- Maintaining clear, objective records of plans of action, progress and recommendations from the client and others.
- Take action to minimize risk and maximize participation/inclusion for individuals well-being (physically, socially, recreationally, educational, personal development, interpersonal and personal relationships).
- Communicating effectively with other members of the individual’s support network in a timely, accurate and respectful way
- Preparing and deliver reports and various documents (i.e. presentations, service plans, month ends, contact notes etc.)
Community Connector / Job Developer
- Helping to identify employment goals, assessing employment readiness and explore job options by working closely with the team and client’s network
- Understanding and utilizing resources and tools available in the community to enhance and develop clients’ employment skills and knowledge
- Establishing and maintain professional relationships with community employers based on mutual respect, trust and honesty by means of dialogue, action and relationship development.
- Work directly with employers and CDSW’s to customize jobs for clients, including identifying available accommodations and adaptations that are required for the individual to do the job
- Providing advocacy support and feedback that addresses the employer ’s concerns regarding hiring, orientation and training for individuals with disabilities, while maintaining client confidentiality. Effectively communicate the benefits of employing people with disabilities in ways that employers relate to.
- Working with clients and CDSW to ensure that they have the proper supplies (e.g., tools, PPE’s) and documentation (e.g., Social Insurance Number) to start work.
- Engaging with the client and the client’s support network in developing and reviewing individualized service plans, so that employment-related plans fit with ongoing support services in other areas of life
- Collaborating with others in the same role, sharing resources and responsibilities
- Assisting in assessing the safety of the work site and the client’s job activities, and address any hazards based on the individual’s unique safety needs. This may include assisting the individual to understand safety rules and training.
- Assisting individuals in understanding their rights in the workplace and supporting them in advocating for themselves when needed
- Analyzing the workplace in order to determine the best means of establishing natural supports. This may include monitoring the workplace social media sites for opportunities. Coaching the clients and CDSW as required to promote the social inclusion of the client in the workplace
- Providing on-the-job support that helps clients learn their job duties and identifying factors that are likely to have an impact on their job success
- Observing the client and others in the workplace (i.e., proactive check-ins) to identify what is working and what is not working and discuss observations with the client and the client’s employer to determine if additional or different support is needed.
