**As the AIWCC Case Management, you will deliver a client-centered case management service model to support newly arrived government-assisted and privately sponsored refugees/newcomers with their settlement in Edmonton. The Case Manager will each hold a case load, comprised of individuals and families, which they will assist to integrate into the community for a period of one to two years. The AIWCC Case Manager provides Case Management tailored to the complexity of the client’s barriers and needs, which includes regular check ins with the clients, appropriate referrals, and follow ups.**

**Primary Duties and Responsibilities include, but are not limited to**:

- Conduct initial intake and needs assessments using the needs assessment matrix, weekly check-ins, empowerment plans, and on-going needs assessments as required
- Provide case management services according to the clients’ level of need through the use of assessments, information orientations and referrals, accompaniments, home visits, and follow-ups and advocacy to assist clients to live independently and integrate successfully using anti-oppressive and empowerment-based practices
- Provide necessary interpretation and translation for direct service delivery with clients at appointments in the community
- Provide information orientations and referrals to a wide array of community agencies, professional services, settlement, and social services, etc. across Edmonton according to the needs of the clients
- Conduct regular home visits/virtual/phone and accompany clients to various appointments in the community for different services and resources
- Provide life skills and crisis intervention as needed
- Liaise, collaborate, and advocate with internal programs and external service providers / community agencies for services and resources in order to bridge the services needed for this population
- Participate in case management meetings with internal staff and external service providers **Coordinate and facilitate program activities as required**:

- Maintain thorough documentation of client cases according to the established case management Apricot software
- Prepare and submit monthly, quarterly, and annual service and statistical reports according to the program and funder requirements
- Assist with the monitoring and evaluation of the program on an on-going basis, recommending service delivery changes and assisting with their implementation when necessary
- Participate in supervision and consultation regularly with the program manager
- Adhere to the policies and procedures of AIWCC
- Attend staff meetings and training sessions as required
- Other duties as needed by supervisor
- In addition to normal duties, from time to time there may be work assignments arising from special events, special projects, or overall program initiatives.

**Qualifications**:

- A post-secondary degree in Social Work, Social Services, Counselling, Psychology, Human Services or related fields, strong preference for Bachelor -level degree in Social Work or related field
- Minimum 2 years’ relevant practical experience working with a relevant population is essential
- **Ability to speak fluently in languages such as Pashto/Dari/Farsi ,Arabic, Somali, Swahili, Ukrainian, French,Tigrinya is a strong asset**:

- Minimum 2-years experience Case Management role serving a vulnerable population, including victims of trauma, is an asset
- Proven ability to work effectively with newcomer clients from diverse cultural backgrounds
- Knowledge of the settlement services and integration process and working knowledge of challenges faced by newcomers in Alberta/Canada
- Extensive knowledge of local community programs, services, and resources
- Excellent communication, organization, time management and coordination skills
- Proven ability to multi-task and thrive in a fast-paced environment and high-stress situations
- Must have a positive and collaborative attitude, demonstrates flexibility, reliability and proactiveness, and takes initiative
- Fluency in oral and written English is required
- Advanced computer skills and strong abilities using the following programs: Microsoft Word, PowerPoint, Excel and Outlook, Adobe, and program database
- Non-violent crisis intervention and First Aid/CPR training are assets
- Available to work flexible / irregular hours including evenings and weekends
- Driver’s license and access to a vehicle for work purposes is a strong asset
- Clear Child intervention, and police clearance
