**POSITION**: Registered Practical Nurse, Withdrawal Management

**LOCATION**: This position is part of the ADAPT hybrid model of operation and is a combination of remote work and in-office/ community (North Halton)

**This position will be classified as a permanent 1.0FTE**

This is a hybrid position consisting of working primarily out of the Milton office/ community and some days of remote work at home.

**SUMMARY**:
The Registered Practical Nurse (RPN) works as a member of the CWMS team, collaborating on the delivery of holistic, comprehensive care to meet the needs of the clients of the CWMS Program. The RPN provides services in the nursing assessment, planning, implementation and evaluation of client care for the purpose of promoting, maintaining or restoring health, and preventing illness, injury and disability. The RPN provides nursing and counselling care to clients throughout the stages of pre-withdrawal, acute withdrawal and post-acute withdrawal.

**ESSENTIAL DUTIES AND RESPONSIBILITIES**:

- Performs nursing assessments as part of the CWMS assessment process, as appropriate
- Maintains accurate, detailed documentation and files, according to established standards
- Assesses and records symptoms and changes in clients' conditions
- Provides recommendations or referrals to appropriate medical care when needed
- Modifies client treatment plans as indicated by clients' conditions
- Monitors all aspects of client care, as required
- Assesses the needs of individual clients, including assessment of individuals' home environments to identify potential risks to the client’s health and safety
- Engages in research and development related to nursing practices as practiced within the CWMS Program at ADAPT
- Makes effective and timely referrals to community resources within the Mississauga/Halton LHIN as required
- Travel across the Halton region

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills, and/or abilities required to enter the position.

**EDUCATION and/or EXPERIENCE**
- Practical Nursing diploma
- Current registration with the College of Nurses of Ontario as a Registered Practical Nurse
- Minimum of 3-5 years of previous nursing experience
- Counselling experience in addiction/concurrent disorders counselling is an asset
- Sound knowledge of psychotropic medication and withdrawal management medication
- Good knowledge of mental health issues and related legislation, specifically Ontario’s Mental Health Act and PHIPA
- Current CPR and First Aid certification is required.

- Ability to effectively present complex information in writing and verbally
- Ability to read and understand documentation such as operating instructions, procedures manuals and complex clinical reports

**REASONING ABILITY**
- Ability to deal effectively with ‘people’ and administrative challenges
- Ability to analyze, prioritize and resolve problems involving a large number of abstract and concrete variables.

**COMPUTER SKILLS**
- Ability to use common office administration software, such as MS Office

**PERSONAL TRANSPORTATION**

Current valid driver's licence and regular access to a dependable vehicle is required.
