**Job Summary - Rehabilitation Coordinator**

iScope Concussion & Pain Clinics is Canada’s only national specialty medical and rehabilitation centre housing a specialist-led multidisciplinary team who are ready to address all aspects of a patient's recovery from concussion and mTBI related conditions, as well as musculoskeletal pain conditions. Our team includes sub-specialized physicians, nurse practitioners, rehabilitation professionals and technologists who are committed to advancing the care of concussion, pain, and neurological disorders. From testing and diagnosing to conducting treatment, we treat all physical, cognitive, emotional, and sleep-related symptoms under one roof.

We are seeking an experienced rehab coordinator that can display compassion and empathy to our patients and lead them through their journey to recovery utilizing the services within our interdisciplinary clinic.

*Responsibilities and Duties*
- Work is on site primarily
- Manage a caseload of new and ongoing brain trauma / concussion and pain patients through their journey to recovery.
- Provide proactive communication to patients and various stakeholders (their legal counsel, their insurer, their employer, their insurance provider, their family doctor) to keep everyone involved in the patient’s recovery apprised of rehabilitation and claim status.
- Partner with internal practitioners to advocate on behalf of the patient and to coordinate rehabilitation.
- Explain the treatments, milestones and expected outcomes of prescribed rehabilitation.
- Support the creation and submission treatment plans that are catered to the patient’s unique needs in partnership with the interdisciplinary treatment providers

*Qualifications and Skills*
- Experience working as an OTA/PTA in an inter-disciplinary clinic for treating head injuries
- Knowledge of the Statutory Accident Benefits Schedule (SABS), familiarity with OCF forms, WSIB and EHC is an asset.
- Experience with MVA claims
- Highly organized multi tasker
- Strong documentation skills
- Effective Communicator
- Occupational/Physical Therapy Assistant (OTA/PTA) diploma
- Basic medical terminology
- English proficiency and communication
- Ability to work under supervision as well as independently
- Punctual and reliable

Training and an effective onboarding will be provided to ensure successful integration into the team

**Job Types**: Full-time, Contract
