**Clinical Intake Coordinator and Community Navigator**

**Position Summary**
- The Intake Coordinator and Community Navigator is the first point of community contact to Hope and Me -MDAO, providing Intake and service coordination, including information, screening, support, navigation, and referral.
- The purpose of the **Clinical Coordinator and Community Navigator position **is to engage, screen, assess, refer, provide brief interventions, and coordinate care for young people, adults, and seniors that contact or are referred to Hope and Me -MDAO.
- **Clinical Intake Coordinator and Community Navigator **will build a positive rapport with a wide range of participants and clients and their families.
- The role provides an opportunity to balance clinical and administrative skills, creating an accessible, responsive, and straightforward way for clients, families, and healthcare providers to access the services they need.
- The position is well-matched to a self-starter with strong organizational and prioritization skills who can work independently and collaborate successfully within a team environment.
- This position liaises with a multidisciplinary team regarding the assignment and monitoring of referrals, participates in regular clinical supervision, and presents and discusses cases with the clinical collaborative care team.

**Key Responsibilities**

**1. Clinical Intake**
- Provides clinical intake services for Hope +Me-MDAO by assessing client-eligibility criteria.
- Works with applicable community partners and services to organize and provide information to community partners.
- Maintains client waitlist and appropriate records.
- Attends relevant staff meetings and committee meetings.
- Retains frequent communication with internal and external partners to ensure seamless and reasonable service flow.
- Follow-up with clients, families, or mental health professionals and support services to communicate recommendations, coordinate implementation of the initial service plan, and referral to other resources.
- Coordinate scheduling assessments, consultations, and client documentation and measures collection.
- Conducts other duties as assigned

**2. Service Navigation and Coordination**
- Sustains a professional network of service contacts to which Hope +Me can refer clients for service outside the Hope +Me-MDAO
- Delivers service navigation and referral support to clients; and
- Maintains updated records and statistics to provide client service needs and outcomes.

**3. Direct Clinical Service**
- Prepares and presents cases at weekly community mental health intake meetings and assigns points to appropriate service providers.
- Provides interim emotional support and psychoeducation to clients and caregivers involved in forensic investigations.
- Participates in regular peer consultation and individual supervision meetings; and
- Maintains clinical records and appropriately records case-related activities following professional regulations/standards and agency policies.

4. **Administration/Quality Improvement**
- Lead Intake related clinical meetings and provide summary notes of decisions.
- Input, track, and monitor electronic database information, coordinating with the data specialist as required.
- Monitor and support clinicians to complete data entry/paperwork.
- Implement intake procedures and evidence-based screening tools as per Hope and Me requirements.

**Knowledge**
- Sound knowledge of trauma-informed clinical best practices in all areas.
- Strong expertise in applicable theories and evidence-based treatment modalities.
- Consistent ability to practice from an anti-oppressive practice framework and demonstrate multicultural competence.
- Current knowledge of best practices in mental health (e.g., crisis de-escalation, suicide prevention, harm reduction, child abuse, and gender-based violence).

**Qualifications and Skills**
- The individual will be skilled in assessing a person’s needs**, including complex risk assessments** and the ability to activate a plan to mitigate any identified risk.
- Excellent interviewing and counselling skills and working knowledge of individual and family assessment and understanding mental health issues in adults, transitional youth, and families.
