TITLE: Community Support Worker - High Needs

**REPORTING RELATIONSHIP**: The Coordinator of Daybreak Program

**QUALIFICATIONS**:A degree or diploma in human/social services field; minimum 2 years experience supporting persons with disabilities; strong advocacy skills; skills that demonstrate leadership, goal setting, problem solving, and organizational skills; valid drivers' licence and required insurance; valid Standard First Aid/CPR Certification, CPI Certification.

**JOB SUMMARY**: In keeping with the philosophy of inclusion and mission of Hutton House, the Community Support Worker will implement leisure activities and provide opportunities for community involvement, education and respite for persons with complex needs in keeping with their aspirations. The Community Support Worker will promote wellness and safety by providing attendant services, administering medications and therapies as required.

**KEY DUTIES AND RESPONSIBILITIES**:
1. Client Supports and Services

Develop, coordinate and implement educational, therapeutic and leisure activities which meet the aspirations of the individual and their family.

Foster partnership with other organizations and caregivers.

Provide direct support and services to individuals with complex needs while participating in educational, therapeutic and leisure activities both on site and in the community.

Monitor programs by maintaining records of participants activities, conducting evaluations and surveys and inputting data into the computer tracking systems.

Provide personal care as per the primary physical demands requirements.

Administer medication, treatment and therapies as required.

2. Working Relationships

Develop promotional literature regarding respite and recreational opportunities.

Adhere to all policies of the organization to ensure a safe supportive work environment.

Participate as an effective team member.

Attend and participate in staff meetings and/or committees and other events of the organization as required.

Keep the Coordinator of Supports for Adults with High Needs informed.

3. Other

Advocate to assure the dignity and the rights of persons with disabilities are advanced.

Engage in professional development.

Participate in a performance evaluation as required.

Other duties as agreed upon.

Contract length: 9 months

Pay: From $23.75 per hour
