**Position Overview**
We are seeking to fill multiple position of a highly motivated **Support Worker II** in a 1 year contract position. In this key role, you will provide direct support to individuals with developmental disabilities. You will also have case management responsibilities, working closely with clients, their families, and other service providers. As a leader on the team, you will help coordinate care and lead a group of dedicated support staff.

**Key Responsibilities**
- Provide direct care and assistance to individuals with developmental disabilities in various settings, fostering independence and skill development.
- Be a part of leading a team of Support Workers, ensuring the delivery of high-quality care and services.
- Develop, implement, and monitor individualized care plans based on client needs, goals, and preferences.
- Act as the case manager for assigned clients, coordinating services and ensuring effective communication between clients, families, service providers, and other stakeholders.
- Conduct regular assessments and reviews of client progress, adjusting care plans as necessary to meet evolving needs.
- Provide mentorship and training to new staff, ensuring they are well-equipped to provide high-quality care.
- Ensure compliance with all relevant policies, regulations, and standards.
- Foster a safe, respectful, and supportive environment for individuals, staff, and families.
- Collaborate with multidisciplinary teams to enhance the well-being and development of clients.

**Qualifications**
- Minimum of 18 months of experience in developmental services or related field with a University Degree or DSW Diploma or acceptable equivalent

**-OR-**
- Diploma in the field of Social and/or Human Services with a minimum of 5 years’ experience in developmental services

**-AND-**
- Knowledge of developmental disabilities and person-centered planning.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team environment.
- Strong organizational and time management skills.
- Valid G driver’s license
- Strong knowledge of AIMS, PAssport/TIMMS, Google suite
- The ability to speak French is an asset
- Valid Vulnerable Sector Police Check required

Pay: $30.97 per hour
