**Position Summary**

Reporting to the Jordan’s Principle Program Manager, the Jordan’s Principle Advocate should possess extensive experience in supporting First Nation children, youth and families. The role requires the ability to work independently and collaboratively within a team environment.

**Key Responsibilities**
- Provide support to First Nation children, and youth needing multiple services and resources by engaging with key contacts in provincial and federal government departments, and other relevant entities.
- Advocate on behalf of First Nation families, and children, who require multiple services and resources.
- Assist clients and families in navigating the resolution of concerns and facilitate referrals to appropriate contacts (federal, provincial) or external resources as necessary.
- Explain healthcare interventions, plans, and educational requirements to clients and families, ensuring comprehension and involvement in care and support planning.
- Facilitate communication of referral information to suitable community organizations.
- Collaborate with and refer clients to appropriate service providers or programs to address issues of concern.
- Coordinate with healthcare and social services disciplines to implement discharge and follow-up plans for identified children and adults (e.g., Nursing Stations, Health Centres, NIHB, Patient Transportation Unit, Home Care, Education, Child and Family Service Agency).
- Maintain confidentiality as per Personal Health Information Act (PHIA standards).
- Perform other duties as assigned by the Jordan’s Principle Program Manager or as needed to support program goals and objectives.

**Qualifications**

**Education & Experience**
- Bachelor’s degree in a related field of Social Sciences (e.g., Social Work, Human Justice, Sociology, Native Studies).
- Alternatively, an equivalent combination of education and a minimum of five (5) years’ direct experience working in child welfare, specifically with First Nation individuals dealing with complex medical conditions and disabilities, may be considered.
- Exceptional verbal, written, and interpersonal communication skills.
- Ability to engage effectively with diverse stakeholders, including healthcare professionals, government officials, and community members.
- Proven experience working sensitively with First Nation individuals, communities, and traditional knowledge systems, and understanding of First Nation health issues.
- Familiarity with available programs and resources for First Nations citizens and communities, both on and off Reserve, including Special Needs and Disability services.
- Knowledge of First Nation communities' culture, traditions, attitudes, and socio-economic conditions.
- Experience in collaborative, integrated program and community planning.
- Proficiency in Client-Centered Case Management as a service delivery approach.
- Strong problem-solving and organizational abilities.
- Proficiency in Microsoft Programs Word, Excel, Outlook.
- Excellent time management skills.
- Ability to work independently and with mínimal supervision.
- Knowledge of First Nations culture and proficiency in a First Nations language would be advantageous.
- Ability to function effectively as part of a team.
- Adherence to AMC policies and procedures, with particular attention to attendance and professional ethics.
- Flexibility to work flexible hours.
- Must have a valid Driver’s License and have access to a reliable vehicle.
- Must have a current satisfactory Criminal Record Check, Clear Child Abuse Checks.
