**AVOIDING DIABETES AFTER PREGNANCY TOGETHER WITH MOMS (ADAPT-M): BRIDGING CARE FROM THE THIRD TO THE FOURTH TRIMESTER**
Together with Women’s College Hospital, the University of Toronto, and the School of Nursing at McMaster University, Dr. Diana Sherifali, Heather M. Arthur Population Health Research Institute/Hamilton Health Sciences Chair in Inter-Professional Health Research, is seeking a post-doctoral fellow to support the adaptation, implementation, and evaluation of a complex health service program. The Avoiding Diabetes after Pregnancy Together with Moms (ADAPT-M) project is an evidence-based virtual health coaching program tailored for patients who have experienced gestational diabetes mellitus (GDM). Delivered by trained certified diabetes educators (CDEs) during the post-partum period, ADAPT-M aims to prevent the onset of diabetes and improve overall health outcomes. Building upon pilot studies and effectiveness trials, the goal of this study is to develop a transformative, evidence-informed, and equitable implementation strategy for ADAPT-M that integrates prenatal to post-partum transition of care and improves quadruple aim outcomes.
To achieve this goal, we have assembled a quadripartite team of researchers, providers, decision-makers, and patients in 3 Ontario settings to co-design, implement, and evaluate the effectiveness of an implementation strategy that is adaptable to local contexts and can be scaled provincially and nationally. Our program will target the quadruple aims by improving population health and healthcare costs through reduced risk of Type 2 Diabetes Mellitus, patient experience by enhancing self-care support for mothers with previous GDM, and provider experience by facilitating the transition of prenatal to postpartum diabetes care. As many marginalized groups are disproportionately affected by GDM, we will advance health equity with a universally accessible program that is culturally adaptable to patient resources and needs.
**Study Aims**
- To co-design an implementation and evaluation strategy for an evidence-informed and equitable health coaching intervention (ADAPT-M program) in 3 Ontario healthcare settings that is focused on transformative integrated care for postpartum GDM patients.
- To iteratively adapt and sequentially implement the ADAPT-M integrated program in each of the 3 Ontario healthcare settings.
- To determine the effectiveness of the ADAPT-M integrated program and implementation strategy in each of the 3 Ontario healthcare settings based on quadruple aim outcomes which will support its provincial and national expansion.
As a co-investigator on this study, the Fellow will:
- Collaborate with research staff and other members of the multi-disciplinary research team to ensure the conduct and successful completion of the study;
- Lead and also contribute to specific agreed-upon components of the intervention adaptation and evaluation process;
- Contribute to additional research-related tasks relevant to the co-design and implementation of the study;
- Attend weekly and ad hoc research team meetings;
- Lead and contribute to research dissemination and knowledge translation activities including meetings with stakeholders, interviews and workshops with multi-disciplinary teams, and dissemination of study results in various forums (e.g., social media, rounds, stakeholder groups, website) and formats (presentations, publications, briefing notes); and
- Participate in educational sessions and contribute to the academic mandate of the participating academic partners through participation in rounds of presentations and the research training of interdisciplinary learners.
The Fellowship experience will be tailored to meet individualized career development goals and clinical, academic, and research learning needs, through but not limited to:
- Mentorship and training by a research supervisor
- Participation in trainee network meetings and rounds
- Participation in regular research rounds and presentations as available
- Development and submission of manuscripts for publication
- Peer support and collaboration with other research trainees
**Required Qualifications**:
- Completion of a PhD in a health or health-related field within the last 5 years
- Demonstrated training and experience in complex health interventions, specifically the design, adaptation, implementation, and evaluation.
- Experience in community-based research.
- Experience with quantitative and qualitative research methods.
- Excellent oral and written communication skills.
- Ability to effectively engage and work collaboratively with a wide range of healthcare stakeholders and study participants.
- Ability to collaborate and work as an effective team member in a multidisciplinary research team.
