**Position Summary**
- To provide comprehensive housing-based case management services to support moderate to high acuity men who are in the process of being discharged from Ottawa Carleton Detention Centre (OCDC) to become housed and clients referred from the Centralized Waiting List
- To promote sustainable improved quality of life by facilitating timely access to community resources necessary to support housing stability and long-term housing retention
- Provide individualized supports tailored to clients' needs
**Qualifications**
**Preferred Experience**:
- Minimum 3 years of engaging and collaborating with vulnerable populations, specifically men who are at-risk or already involved in the justice system
- Past or present positions held within the youth or adult justice system (institutional clearance considered an additional asset)
- Past or present positions successfully collaborating with the homeless and housing sector in Ottawa
- Proven success in collaborating with system partners in multi-sectoral roles
- Training and experience in the VI-SPDAT and other assessment tools
- Experience working within a Housing First framework and its guiding principles
- Current Non-Violent Crisis Intervention and ASIST certification preferred, and advanced MI training considered a strong asset
**Education**:
- University Degree/College Diploma in Social Services or equivalent
**Essential Skills**:
- Understanding of factors that contribute to justice system involvement for adult males
- Strong Motivational Interviewing skills to support the establishment of case plans with clients, as well as support them in taking steps to achieve those goals
- Competency using crisis management skills
- Strong organizational skills to support Coordinator with accomplishing various tasks set out in the HBCM work plan
- Competency in report writing, gathering, and interpreting data and outcome measures, as required by Coordinator
- Understanding of operating within agency policies and procedures
- Have a demonstrated ability to effectively engage male clients
- Have a demonstrated ability to work within timelines and deadlines
- Possess strong time management and organizational skills
- Have their own reliable vehicle for community meetings with clients and collaterals
- Have excellent people skills to facilitate relationships with clients, colleagues, supervisors, program partners and community members
- Have a genuine interest in working with men in a caring and empathic manner
- Be willing to work evenings and weekends as required
- Bilingualism considered a strong asset
**Responsibilities**
The Justice-Involved HBCM will:
- Support a caseload of approximately 15-20 adult men who have a history of repeated incarceration that has resulted in episodic and chronic homelessness. The clients may have mental health and substance use issues, acquired brain injury and trauma related behaviours
**Service Provision -This position will provide the following services**:
**Intake and Assessment**:
- Identify clients by in-reach activities at Ottawa-Carleton Detention Centre and assess for HBCM services via a Justice Discharged VI-SPDAT (JD VI-SPDAT) and case-conferencing with OCDC discharge planners
- Forward potential client names that meet the eligibility criteria
- Liaise with the City’s HF Data Coordinator to identify and prioritize clients
